摘要:
目的 探讨布比卡因加小剂量吗啡行收肌管阻滞治疗全膝关节置换术后疼痛的有效性及安全性。方法 纳入2019年9月~2020年7月于安徽医科大学第一附属医院行择期膝关节置换的患者62例，采用随机数字表法将入组患者分为布比卡因组（GB组）与吗啡组（GM组），各31例。GB组以0.375%布比卡因20 ml行收肌管阻滞，GM组使用0.375%布比卡因加3 mg吗啡共20 ml行收肌管阻滞。比较两组拔管后即刻与术后6、12、24、48 h的视觉模拟疼痛评分（VAS评分）、术后阿片类药物消耗量、肌力及术后并发症的发生率。结果 GM组在术后6、12、24 h的VAS评分低于布比卡因组，差异有统计学意义（P＜0.05），两组48 h的VAS评分比较，差异无统计学意义（P＞0.05）；GM组术后第1天阿片类药物消耗为（10.01±4.83）mg，低于GB组的（13.54±5.51）mg，差异有统计学意义（P＜0.05），两组术后肌力、术后并发症的发生率比较，差异无统计学意义（P＞0.05）。结论 在膝关节置换术后使用布比卡因加吗啡行收肌管阻滞术后镇痛效果优于单纯布比卡因，且不影响下肢肌力，不增加术后并发症。
Abstract:
Objective To investigate the effectiveness and safety of bupivacaine plus low-dose morphine for adductor tube block in the treatment of pain after total knee arthroplasty.Methods 62 patients who underwent elective knee arthroplasty in the First Affiliated Hospital of Anhui Medical University from September 2019 to July 2020 were enrolled. The random number table method was used to divide the enrolled patients into bupivacaine group (GB group) and Morphine group (GM group), 31 cases in each group.Adductor tube block was performed with 0.375% bupivacaine 20 ml in the GB group, and 20 ml with 0.375% bupivacaine plus 3 mg morphine in the GM group.The visual analogue pain score (VAS score), postoperative opioid consumption, muscle strength and the incidence of postoperative complications were compared between the two groups immediately after extubation and at 6, 12, 24, and 48 h after surgery.Results The VAS score of the GM group at 6, 12, and 24 h after surgery was lower than that of the bupivacaine group,the difference was statistically significant (P<0.05).There was no statistically significant difference in the 48-hour VAS scores between the two groups (P>0.05);The consumption of opioids on the first day after operation in the GM group was (10.01±4.83) mg, which was lower than the (13.54±5.51) mg in the GB group,the difference was statistically significant (P<0.05).There was no significant difference in the incidence of postoperative muscle strength and postoperative complications between the two groups (P>0.05).Conclusion The analgesic effect of bupivacaine plus morphine for adductor tube block after knee arthroplasty is better than bupivacaine alone, and does not affect the muscle strength of the lower limbs and does not increase postoperative complications.
