目的 通过声触诊组织定量技术(VTQ)检测高血压肾病(HN)患者双侧肾脏各部位剪切波速度值(SWV)的变化,分析不同眼底病变程度的高血压肾病肾脏剪切波弹性表现。方法 根据Keith-Wagener眼底分级法将收集到的60例HN患者分为KWⅠ组18例,KWⅡ组20例,KWⅢ组22例;另选30例健康体检者为对照组。依次对各组肾脏进行VTQ检查,比较HN患者眼底分级不同组肾皮质、髓质、肾窦部的SWV变化。结果 对照组及HN患者双肾间各部位SWV值比较,差异无统计学意义(P>0.05);在皮质区及髓质区,不同眼底分级组的SWV值均低于对照组,KWⅢ组的SWV值低于KWⅠ组及KWⅡ组(P<0.05);KWⅠ组与KWⅡ组间SWV值比较,差异无统计学意义(P>0.05);对照组及所有HN患者肾窦部的SWV值均无明显变化。HN患者肾脏皮质及髓质的SWV值与其眼底病变严重程度均呈负相关(r=-0.473、-0.425,P<0.05);肾窦部SWV值与眼底病变程度不相关(r=-0.237,P>0.05)。结论 肾脏剪切波速度可反映高血压肾病肾组织弹性变化,并与眼底病变程度存在一定的相关性,对高血压肾损伤有一定的评估价值。
Abstract:
Objective To detect the changes in the shear wave velocity (SWV) of each part of the kidneys in patients with hypertensive nephropathy (HN) through acoustic palpation tissue quantification (VTQ), and to analyze the renal shear wave elasticity of hypertensive nephropathy with different degrees of fundus lesions.Methods According to the Keith-Wagener fundus grading method, the collected 60 HN patients were divided into KWⅠ group with 18 cases, KWⅡ group with 20 cases, and KWⅢ group with 22 cases. Another 30 healthy physical examination subjects were used as the control group. The kidneys of each group were examined in turn for VTQ, and the SWV changes of renal cortex, medulla, and renal sinuses in different groups of HN patients with fundus grading were compared.Results There was no statistically significant difference in SWV values of each part of the kidneys between the control group and HN patients (P>0.05);In the cortex and medulla, the SWV values of the different fundus grading groups were lower than those of the control group,the SWV value of KWⅢ group was lower than that of KWⅠ group and KWⅡ group (P<0.05);There was no significant difference in SWV value between KWⅠ group and KWⅡ group (P>0.05);The SWV value of renal sinus in the control group and all HN patients had no significant changes. The SWV values of renal cortex and medulla of HN patients were negatively correlated with the severity of fundus lesions (r=-0.473, -0.425, P<0.05);The SWV value of renal sinus was not correlated with the degree of fundus disease (r=-0.237, P>0.05).Conclusion Kidney shear wave velocity can reflect the changes in renal tissue elasticity of hypertensive nephropathy, and has a certain correlation with the degree of ocular fundus lesions, and has a certain evaluation value for hypertensive renal injury.
