摘要:
目的 评估3.0T磁共振扩散张量成像(DTI)与扩散加权成像(DWI)对大脑半球胶质瘤病理分级的诊断效能。方法 回顾性分析2016年3月-2021年5月我院神经外科经手术病理证实的52例大脑半球胶质瘤患者的术前MRI平扫、DTI、DWI影像资料与术后病理资料,按胶质瘤病理分级分为高级别组27例和低级别组25例,比较两组瘤体及瘤周MRI表观弥散系数(ADC)、平均弥散系数(MD)及各项异性分数(FA)值,分析两组瘤体及瘤周ADC值、MD值、FA值与病理分级的相关性,并绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),确定诊断阈值,评估DTI及DWI诊断脑胶质瘤病理分级的敏感度和特异性。结果 低级别组瘤体的ADC与MD值高于高级别组,差异有统计学意义(P<0.05);两组间瘤体FA值,瘤周ADC、MD及FA值比较,差异均无统计学意义(P>0.05);两组瘤体ADC、MD值均与病理分级呈负相关(r=-0.566、-0.667,P<0.05);ROC曲线显示,瘤体MD、ADC值鉴别诊断高、低级别脑胶质瘤的AUC分别为0.914和0.851,其对应的鉴别诊断敏感度、特异度及诊断阈值分别为84.00%、96.30%、1.499与76.00%、85.20%、1.537。结论 DTI与DWI均有助于大脑半球胶质瘤的术前分级诊断,且MD值的诊断效能更高。
Abstract:
Objective To evaluate the diagnostic efficacy of 3.0T magnetic resonance diffusion tensor imaging (DTI) and diffusion weighted imaging (DWI) in pathological grading of cerebral glioma.Methods The preoperative MRI plain scan, DTI, DWI images and postoperative pathological data of 52 patients with cerebral hemispheric glioma confirmed by surgery and pathology in neurosurgery department of our hospital from March 2016 to May 2021 were retrospectively analyzed. According to the pathological classification of glioma, they were divided into high-level group (27 cases) and low-level group (25 cases). The apparent diffusion coefficient (ADC), average diffusion coefficient (MD), and anisotropy score (FA) values of MRI in the tumor body and tumor circumference of the two groups were compared. The correlation between the ADC value, MD value, and FA value of tumor body and tumor circumference of the two groups and the pathological classification was analyzed, and the receiver operating characteristic (ROC) curve was plotted. The area under the curve (AUC) was calculated, the diagnostic threshold was determined, and the sensitivity and specificity of DTI and DWI in the diagnosis of pathological grading of glioma were evaluated.Results The ADC and MD values of the low-grade group were higher than those of the high-grade group, and the difference was statistically significant (P<0.05). There was no significant difference in tumor FA value, ADC, MD and FA value between the two groups (P>0.05). ADC and MD values of the two groups were negatively correlated with pathological grade (r=-0.566, -0.667, P<0.05). ROC curve showed that the AUC of MD and ADC values in the differential diagnosis of high-grade and low-grade gliomas were 0.914 and 0.851, respectively; the corresponding diagnostic sensitivity, specificity and diagnostic threshold were 84.00%, 96.30%, 1.499 and 76.00%, 85.20%, 1.537, respectively.Conclusion Both DTI and DWI are helpful for the preoperative grading diagnosis of hemispheric glioma, and the diagnostic efficiency of MD value is higher.
