摘要:
目的 观察锁定接骨板治疗累及肱骨距的肱骨近端骨折的临床效果，分析其对并发症和功能恢复的影响。方法 选取2020年3月-2021年4月在我院诊治的64例累及肱骨距的肱骨近端骨折患者为研究对象，采用随机数字表法分为对照组和观察组，各32例。对照组采用切开内固定术治疗，观察组采用锁定接骨板治疗，比较两组临床手术指标、骨折恢复优良率、疼痛（VAS）评分、肩关节功能Constant-Mur1ey 评分及并发症发生情况。结果 观察组手术时间、术中出血量、切口长度、骨折愈合时间均短于对照组（P＜0.05）；观察组骨折恢复优良率为93.75%，高于对照组84.37%（P＜0.05）；观察组疼痛评分低于对照组（P＜0.05）；观察组肩关节功能Constant-Mur1ey 各维度评分均高于对照组（P＜0.05）；观察组并发症发生率为6.25%，低于对照组15.63%（P＜0.05）。结论 锁定接骨板治疗累及肱骨距的肱骨近端骨折效果理想，手术时间短、术中出血量少。同时可确保有效支撑，提高骨折恢复优良率和肩关节功能，患者疼痛轻，并发症少，值得临床应用。
Abstract:
Objective To observe the clinical effect of locking plate in the treatment of proximal humeral fractures involving humeral distance, and to analyze its effect on complications and functional recovery.Methods From March 2020 to April 2021, 64 patients with proximal humeral fractures involving the humeral distance in our hospital were selected as the research subjects. They were divided into control group and observation group by random number table method, with 32 cases in each group. The control group was treated with open internal fixation, and the observation group was treated with locking plate. The clinical operation indexes, excellent and good rate of fracture recovery, VAS score, Constant-Mur1ey score of shoulder joint function and complications were compared between the two groups.Results The operation time, intraoperative blood loss, incision length and fracture healing time of the observation group were shorter than those of the control group (P<0.05). The excellent and good rate of fracture recovery in the observation group was 93.75%, which was higher than 84.37% in the control group (P<0.05). The pain score in the observation group was lower than that in the control group (P<0.05). The Constant-Mur1ey scores of shoulder joint function in the observation group were higher than those in the control group (P<0.05). The incidence of complications in the observation group was 6.25%, which was lower than 15.63% in the control group (P<0.05).Conclusion The effect of locking plate in the treatment of proximal humeral fractures involving humeral distance is ideal, with short operation time and less intraoperative bleeding. At the same time, it can ensure effective support, improve the excellent and good rate of fracture recovery and shoulder joint function, and patients have less pain and fewer complications, which is worthy of clinical application.
