摘要:
目的 探讨中青年冠心病经皮冠状动脉介入治疗（PCI）术患者的配偶照顾者疾病获益感现状，并分析其影响因素。方法 采用便利抽样法选取2021年6月-12月在我市某三级甲等医院心血管内科住院的337名中青年PCI患者的配偶照顾者为研究对象，采用一般资料问卷、照顾者疾病获益感问卷、家庭关怀度问卷、一般自我效能感问卷和简易应对方式问卷对其进行横断面调查。结果 337名配偶的照顾者疾病获益感总分为（115.10±14.80）分，家庭关怀度得分（8.25±1.91）分，自我效能得分（27.30±7.71）分，积极应对得分（24.55±6.73）分，消极应对得分（11.79±5.78）分。相关分析显示，疾病获益感与家庭关怀度（r=0.141，P=0.010）、自我效能（r=0.324，P=0.000）和积极应对（r=0.283，P=0.000）得分均呈正相关。回归分析显示，照顾者文化程度、与患者感情、患者心功能分级、家庭关怀度、自我效能及积极应对方式是照顾者疾病获益感的影响因素（P＜0.05）。结论 中青年PCI患者的照顾者疾病获益感处于中等以上水平，应从提高其家庭功能、自我效能感和应对方式入手，考虑照顾者的文化程度、与患者的关系及患者的疾病严重程度等特点，制定个性化的干预方案，以提高其疾病获益感水平。
Abstract:
Objective To explore the status quo of perceived caregiving benefits of spouse caregivers for young and middle-aged patients with coronary heart disease treated with percutaneous coronary intervention (PCI), and analyze its influencing factors.Methods The convenience sampling method was used to select 337 spouse caregivers of young and middle-aged PCI patients who were hospitalized in the cardiovascular department of a tertiary hospital in our city from June to December 2021. The general information questionnaire, perceived caregiving benefits questionnaire, the family care questionnaire, the general self-efficacy questionnaire and the simple coping style questionnaire were used to conduct a cross-sectional survey.Results The total score of perceived caregiving benefit of 337 spouse caregivers was (115.10±14.80) scores, the score of family care was (8.25±1.91) scores, the score of self-efficacy was (27.30±7.71) scores, the score of positive coping was (24.55±6.73) scores, and the score of negative coping was (11.79±5.78) scores. Correlation analysis showed that perceived caregiving benefit was positively correlated with family care (r=0.141, P=0.010), self-efficacy (r=0.324, P=0.000) and positive coping (r=0.283, P=0.000). Regression analysis showed that spouse caregivers’ education level, feelings with patients, cardiac function classification, family care, self-efficacy and positive coping style were the influencing factors of spouse caregivers’ perceived caregiving benefit (P<0.05).Conclusion The level of perceived caregiving benefits of spouse caregivers for young and middle-aged patients after PCI is above the medium level. It is necessary to improve their family function, self-efficacy and coping style, consider the characteristics of spouse caregivers’ education level, relationship with patients and the severity of patients’ disease, and formulate personalized intervention programs to improve their level of perceived caregiving benefits.
