摘要:
目的 比较3D与2D腔镜系统对早期肺癌患者手术的疗效,探讨3D胸腔镜技术的安全性及应用价值。方法 回顾性分析我科2018年1月-2021年12月接受电视胸腔镜手术(VATS)的早期肺癌患者,并以术中使用不同的腔镜系统分为3D-VATS组(n=34)和2D-VATS组(n=80)。比较两组手术时间、术中出血量、术后引流量,以及清扫淋巴结数目和术后并发症等手术疗效指标。结果 3D-VATS组手术时间短于2D-VATS组(P<0.05),术中出血量、术后总引流量、总住院时间少于2D-VATS组(P<0.05);两组淋巴结清扫数目、中转开胸率、术后出现肺漏气时间延长、支气管胸膜瘘、肺部感染、心律失常等并发症发生率比较,差异无统计学意义(P>0.05)。结论 3D-VATS比传统2D-VATS在早期肺癌手术中具有更好的疗效,3D胸腔镜技术是安全、可靠的。
Abstract:
Objective To compare the efficacy of 3D and 2D thoracoscopic surgery in patients with early-stage lung cancer, and to explore the safety and application value of 3D thoracoscopic surgery.Methods A retrospective analysis was performed on patients with early-stage lung cancer who underwent video-assisted thoracoscopic surgery (VATS) in our department from January 2018 to December 2021. The patients were divided into 3D-VATS (n=34) and 2D-VATS (n=80) according to different endoscopic systems used during operation. The operation time, intraoperative blood loss, postoperative drainage, number of lymph nodes dissected and postoperative complications were compared between the two groups.Results The operation time of the 3D-VATS group was shorter than that of the 2D-VATS group (P<0.05 ), and the intraoperative blood loss, total postoperative drainage volume and total hospitalization time were less than those of the 2D-VATS group (P<0.05). There was no significant difference in the number of lymph node dissection, the rate of conversion to thoracotomy, the prolongation of postoperative pulmonary leakage time, bronchopleural fistula, pulmonary infection, arrhythmia and other complications between the two groups (P>0.05).Conclusion Compared with traditional 2D-VATS, 3D-VATS has better efficacy in early-stage lung cancer surgery, and 3D thoracoscopy is safe and reliable.
