摘要:
目的 探究卡前列甲酯栓联合麦角新碱对瘢痕子宫（SU）剖宫产患者产后出血（PPH）的预防效果及安全性。方法 以2020年10月-2022年10月我院拟行剖宫产分娩的60例SU产妇为研究对象，依据随机数字表法分为对照组（30例）与观察组（30例）。对照组于胎儿娩出后采用卡前列甲酯栓舌下给药，观察组在其基础上联合麦角新碱子宫颈注射，比较两组PPH预防效果、产后出血量（术后2、24 h）、血常规[血红蛋白（Hb）及红细胞压积（HCT）]、产后恢复情况（术后住院时间、恶露持续时间、子宫底下降速度）、不良反应。结果 观察组PPH预防总有效率高于对照组（P＜0.05）；观察组术后2、24 h出血量均少于对照组（P＜0.05）；两组术后HCT低于术前（P＜0.05），但组间比较，差异无统计学意义（P＞0.05）；两组术后Hb低于术前，但观察组Hb高于对照组（P＜0.05）；观察组术后住院时间、恶露持续时间短于对照组，且子宫底下降速度大于对照组（P＜0.05）；两组不良反应发生率比较，差异无统计学意义（P＞0.05）。结论 卡前列甲酯栓联合麦角新碱对SU剖宫产产妇的PPH风险具有良好预防效果，可降低其产后出血量，减少血红蛋白流失，加速子宫复旧，且用药安全性高。
Abstract:
Objective To explore the preventive effect and safety of methyl carprost suppositories combined with ergometrine on postpartum hemorrhage (PPH) in cesarean section patients with scar uterus (SU).Methods From October 2020 to October 2022, 60 pregnant women with SU who planned to undergo cesarean section in our hospital were selected as the research objects. According to the random number table method, they were divided into control group (30 pregnant women) and observation group (30 pregnant women). The control group was given sublingual administration of methyl carprost suppositories after delivery, and the observation group was combined with ergometrine cervical injection on the basis of the control group. The preventive effect of PPH, postpartum hemorrhage (2 h and 24 h after operation), blood routine [hemoglobin (Hb) and hematocrit (HCT)], postpartum recovery (postoperative hospitalization time, duration of lochia, decline rate of uterine fundus) and adverse reactions were compared between the two groups.Results The total effective rate of PPH prevention in the observation group was higher than that in the control group (P<0.05). The amount of bleeding at 2 and 24 h after operation in the observation group was less than that in the control group (P<0.05). The HCT of the two groups after operation was lower than that before operation (P<0.05), but there was no significant difference between the two groups (P>0.05). The Hb of the two groups after operation was lower than that before operation, but the Hb of the observation group was higher than that of the control group (P<0.05). The postoperative hospitalization time and duration of lochia in the observation group were shorter than those in the control group, and the decline rate of uterine fundus was greater than that in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Methyl carprost suppositories combined with ergometrine has a good preventive effect on PPH risk in women with SU cesarean section, which can reduce the amount of postpartum hemorrhage, reduce the loss of hemoglobin, accelerate uterine involution, and have high drug safety.
