摘要:
目的 观察静脉输注硫酸镁对经尿道前列腺电切术(TURP)患者术后膀胱痉挛和早期恢复质量的影响。方法 选择2023年4月-7月我院择期行经尿道前列腺电切术患者100例。将患者随机分为硫酸镁组(Mg组,n=50)和生理盐水组(NS组,n=50)。Mg组在麻醉前以15 mg/(kg·h)的速度静脉输注硫酸镁至手术结束,NS组以生理盐水代替MgSO4。比较两组术后48 h内膀胱痉挛的发生率、PCIA泵按压次数以及补救镇痛情况,术前1天及术后1、2天康复质量评分量表(QoR-15)评分,术后6、12、24、48 h时VAS评分,术中及术后不良反应的发生情况,以及术前和术毕的血清镁离子浓度。结果 Mg组术后48 h内膀胱痉挛发生率、PCIA泵按压次数和补救镇痛比例低于NS组(P<0.05);Mg组术后第1和2天QoR-15量表中疼痛、身体舒适度、心理支持、情绪状态评分和总分均高于NS组(P<0.05);Mg组术后6、12、24、48 h VAS评分低于NS组(P<0.05);两组恶心、呕吐、低血压发生率比较,差异无统计学意义(P>0.05);Mg组术毕血清镁离子浓度高于NS组(P<0.05)。结论 静脉输注硫酸镁可以降低术后膀胱痉挛的发生率,促进患者早期康复。
Abstract:
Objective To observe the effects of intravenous infusion of magnesium sulfate on postoperative bladder spasm and early recovery quality in patients undergoing transurethral resection of the prostate (TURP).Methods From April to July 2023, 100 patients undergoing transurethral resection of the prostate were selected. The patients were randomly divided into magnesium sulfate group (Mg group, n=50) and normal saline group (NS group, n=50). In the Mg group, magnesium sulfate was intravenously infused at a rate of 15 mg/(kg·h) before anesthesia until the end of surgery. In the NS group, normal saline was used instead of MgSO4. The incidence of bladder spasm within 48 h after operation, the number of PCIA pump compressions and remedial analgesia were compared between the two groups. The quality of recovery scale (QoR-15) scores at 1 day before operation, 1 day and 2 days after operation, the VAS scores at 6, 12, 24 and 48 h after operation, the incidence of adverse reactions during and after operation, and the serum magnesium ion concentration before and after operation were compared between the two groups.Results The incidence of bladder spasm within 48 h after operation, the number of PCIA pump compressions and the proportion of remedial analgesia in the Mg group were lower than those in the NS group (P<0.05). The scores of pain, physical comfort, psychological support, emotional state and total score of QoR-15 scale in Mg group were higher than those in NS group on 1st and 2nd days after operation (P<0.05). The VAS scoresat 6, 12, 24 and 48 h after operation in the Mg group was lower than those in the NS group (P<0.05). There was no significant difference in the incidence of nausea, vomiting and hypotension between the two groups (P>0.05). The serum magnesium concentration in the Mg group was higher than that in the NS group (P<0.05).Conclusion Intravenous infusion of magnesium sulfate can reduce the incidence of postoperative bladder spasm and promote early rehabilitation of patients.
