摘要:
目的 研究股骨近端防旋髓内钉（PFNA）内固定治疗对老年髋部骨折患者关节活动度及平衡能力的影响。方法 选取2021年6月-2023年8月井冈山市人民医院收治的60例老年髋部骨折患者，经随机数字表法分为对照组（30例）与观察组（30例）。对照组行股骨近端锁定解剖板（LPFP）内固定治疗，观察组则采用PFNA内固定治疗，比较两组手术相关指标（手术时间、下床负重时间、术后住院时间、骨折愈合时间）、关节活动度（髋关节屈曲活动度、髋关节外展活动度）、Harris髋关节功能评分、Berg平衡量表（BBS）评分、并发症发生情况。结果 观察组手术时间、下床负重时间、术后住院时间、骨折愈合时间短于对照组（P＜0.05）。治疗后2个月，两组髋关节屈曲活动度、髋关节外展活动度均大于治疗前，且观察组髋关节屈曲活动度、髋关节外展活动度大于对照组（P＜0.05）。治疗后2个月，两组Harris髋关节功能评分、BBS评分均高于治疗前，且观察组Harris髋关节功能评分、BBS评分高于对照组（P＜0.05）。观察组手术并发症发生率低于对照组（P＜0.05）。结论 PFNA内固定在老年髋部骨折治疗中具有确切应用价值，其手术时间短、术后恢复快、手术并发症少，可改善患者的关节活动度与平衡能力，促进其髋关节功能恢复。
Abstract:
Objective To study the effect of proximal femoral nail antirotation (PFNA) internal fixation on range of motion and balance in elderly patients with hip fracture.Methods Sixty elderly patients with hip fracture admitted to Jinggangshan People’s Hospital from June 2021 to August 2023 were selected and divided into control group (30 patients) and observation group (30 patients) by random number table method. The control group was treated with locking proximal femoral bone plate (LPFP) internal fixation, while the observation group was treated with PFNA internal fixation. The operation-related indicators (operation time, off-bed weight-bearing time, postoperative hospital stay, fracture healing time), range of motion (hip flexion activity, hip abduction activity), Harris hip function score, Berg Balance Scale (BBS) score, and complications were compared between the two groups.Results The operation time, off-bed weight-bearing time, postoperative hospitalization time and fracture healing time of the observation group were shorter than those of the control group (P<0.05). At 2 months after treatment, the hip flexion activity and hip abduction activity of the two groups were greater than those before treatment, and the hip flexion activity and hip abduction activity of the observation group were greater than those of the control group (P<0.05). At 2 months after treatment, the Harris hip function score and BBS score of the two groups were higher than those before treatment, and the Harris hip function score and BBS score of the observation group were higher than those of the control group (P<0.05). The incidence of surgical complications in the observation group was lower than that in the control group (P<0.05).Conclusion PFNA internal fixation has definite application value in the treatment of hip fracture in the elderly. It has short operation time, fast postoperative recovery and few surgical complications. Meanwhile, it can improve the joint mobility and balance ability of patients and promote the recovery of hip joint function.
