摘要:
目的 探讨桃红四物汤合五苓散加减治疗心衰水肿的临床效果及对预后的影响。方法 选取2021年1月-2023年1月万年县中医院诊治的心衰水肿患者60例作为研究对象，将所有患者依据随机数字表法分为研究组和对照组，每组30例。对照组患者予以西药治疗，研究组患者采用桃红四物汤合五苓散加减治疗，比较两组患者的临床治疗总有效率、相关临床治疗观察指标[动脉血氧分压（PaO2）、血氧饱和度（SaO2）、二氧化碳分压（PCO2）、心率（HR）、血清N末端钠尿肽原（NT-ProBNP）]、心功能[左室收缩末期内径（LVESD）、左心室舒张末期内径（LVDD）、心脏射血分数（EF值）]，以及不良反应（头痛、嗜睡、头晕、低钾血症、胃肠道反应）发生率。结果 研究组治疗总有效率高于对照组（P＜0.05）；研究组治疗后SaO2、PaO2、PCO2水平高于对照组，研究组NT-ProBNP、HR水平低于对照组（P＜0.05）；研究组治疗后LVDD、LVESD水平低于对照组，EF值高于对照组（P＜0.05）；研究组不良反应发生率低于对照组（P＜0.05）。结论 桃红四物汤合五苓散加减治疗用于心衰水肿患者临床治疗，能够提升患者的临床治疗成效，改善患者治疗相关临床观察指标及其心功能，减少不良反应的发生。
Abstract:
Objective To explore the clinical effect of Taohong Siwu decoction combined with Wuling powder in the treatment of heart failure edema and its influence on prognosis.Methods A total of 60 patients with heart failure and edema diagnosed and treated in Wannian County Hospital of Traditional Chinese Medicine from January 2021 to January 2023 were selected as the research objects. All patients were divided into study group and control group according to the random number table method, with 30 patients in each group. The patients in the control group were treated with western medicine, and the patients in the study group were treated with Taohong Siwu decoction and Wuling powder. The total effective rate of clinical treatment, related clinical treatment observation indexes [arterial partial pressure of oxygen (PaO2), blood oxygen saturation (SaO2), partial pressure of carbon dioxide (PCO2), heart rate (HR), serum N-terminal natriuretic peptide (NT-ProBNP)], cardiac function [left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVDD), cardiac ejection fraction (EF value)] and adverse reactions (headache, drowsiness, dizziness, hypokalemia, gastrointestinal reaction) were compared between the two groups.Results The total effective rate of treatment in the study group was higher than that in the control group (P<0.05). After treatment, the levels of SaO2, PaO2 and PCO2 in the study group were higher than those in the control group, and the levels of NT-ProBNP and HR in the study group were lower than those in the control group (P<0.05). After treatment, the levels of LVDD and LVESD in the study group were lower than those in the control group, and the EF value was higher than that in the control group (P<0.05). The incidence of adverse reactions in the study group was lower than that in the control group (P<0.05).Conclusion Taohong Siwu decoction combined with Wuling powder in the treatment of patients with heart failure and edema can improve the clinical treatment effect of patients, improve the clinical observation indexes and cardiac function of patients, and reduce the occurrence of adverse reactions.
