摘要:
目的 探讨有创机械通气联合高流量氧疗序贯治疗Ⅱ型呼吸衰竭的效果。方法 选取2021年6月-2023年10月弋阳县人民医院收治并诊断为Ⅱ型呼吸衰竭的68例患者作为研究对象,按照随机数字表法分为对照组和研究组,每组30例。对照组进行有创机械通气治疗,研究组进行有创机械通气联合高流量氧疗序贯治疗。比较两组治疗效果、呼吸循环指标[氧分压(PaO2)、二氧化碳分压(PaCO2)、氧合指数(PaO2/FiO2)、平均动脉压(MAP)、心率(HR)]、肺功能指标 [用力肺活量(FVC)、最大吸气后1 s呼出最大量(FEV1)、最高呼气流速(PEF)]、炎性因子水平[核转录因子-κB(NF-κB)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)]。结果 研究组总有效率高于对照组(P<0.05);研究组PaO2、PaO2/FiO2水平高于对照组,PaCO2、MAP、HR水平低于对照组(P<0.05);研究组FVC、FEV1、PEF水平高于对照组(P<0.05);研究组IL-6、NF-κB、TNF-α水平低于对照组(P<0.05)。结论 有创机械通气联合高流量氧疗序贯治疗Ⅱ型呼吸衰竭的疗效确切,可以改善患者血气指标、肺功能指标以及炎性因子水平。
Abstract:
Objective To investigate the effect of invasive mechanical ventilation combined with high-flow oxygen therapy in the sequential treatment of type Ⅱrespiratory failure.Methods A total of 68 patients with type Ⅱ respiratory failure admitted to Yiyang County People’s Hospital from June 2021 to October 2023 were selected as the research objects. According to the random number table method, they were divided into control group and study group, with 30 patients in each group. The control group was treated with invasive mechanical ventilation, and the study group was treated with invasive mechanical ventilation combined with high-flow oxygen therapy. The treatment effect, respiratory and circulatory indexes [partial pressure of oxygen (PaO2), partial pressure of carbon dioxide (PaCO2), oxygenation index (PaO2/FiO2), mean arterial pressure (MAP), heart rate (HR)], lung function indexes [forced vital capacity (FVC), maximum expiratory volume at 1 s after maximum inspiratory (FEV1), maximum expiratory flow rate (PEF)], inflammatory factor levels [nuclear transcription factor-κB (NF-κB), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6)] were compared between the two groups.Results The total effective rate of the study group was higher than that of the control group (P<0.05). The levels of PaO2 and PaO2/FiO2 in the study group were higher than those in the control group, and the levels of PaCO2, MAP and HR were lower than those in the control group (P<0.05). The levels of FVC, FEV1 and PEF in the study group were higher than those in the control group (P<0.05). The levels of IL-6, NF-κB and TNF-α in the study group were lower than those in the control group (P<0.05).Conclusion Invasive mechanical ventilation combined with high-flow oxygen therapy is effective in the sequential treatment of type Ⅱ respiratory failure, which can improve the blood gas index, lung function index and inflammatory factor level of patients.
