摘要:
目的 研究氯雷他定联合玉屏风颗粒治疗慢性荨麻疹的有效率及对临床症状的影响。方法 选取2022年4月-2023年6月吉安县人民医院治疗的108例慢性荨麻疹患者，采取随机数字表法分为对照组（n=58）与观察组（n=50）。对照组应用氯雷他定治疗，观察组应用氯雷他定联合玉屏风颗粒治疗。观察两组治疗有效率、复发率、血清炎症因子水平[白细胞介素（IL）-4、免疫球蛋白E（IgE）、IL-8、干扰素-γ（INF-γ）和IL-17]、症状改善程度[风团最大直径评分、风团数目评分、每日风团发作次数评分、瘙痒程度评分和风团持续时间评分]，以及细胞免疫变化[CD4+、CD8+、CD4+/CD8+]。结果 观察组治疗有效率高于对照组，复发率低于对照组（P＜0.05）。两组治疗后血清IL-4、IgE、IL-8和IL-17水平均低于治疗前，INF-γ水平高于治疗前，且观察组血清IL-4、IgE、IL-8和IL-17水平低于对照组，血清INF-γ水平高于对照组（P＜0.05）。两组治疗后风团最大直径评分、风团数目评分、每日风团发作次数评分、瘙痒程度评分和风团持续时间评分均低于治疗前，且观察组风团最大直径评分、风团数目评分、每日风团发作次数评分、瘙痒程度评分和风团持续时间评分均低于对照组（P＜0.05）。两组治疗后CD8+水平均低于治疗前，CD4+、CD4+/CD8+水平高于治疗前，且观察组CD8+水平低于对照组，且CD4+、CD4+/CD8+水平高于对照组（P＜0.05）。结论 氯雷他定联合玉屏风颗粒治疗对慢性荨麻疹有确切的疗效，能够改善患者临床症状及细胞免疫功能，抑制患者机体炎性反应，降低患者复发率，值得临床应用。
Abstract:
Objective To study the effective rate of loratadine combined with Yupingfeng granule in the treatment of chronic urticaria and its effect on clinical symptoms.Methods A total of 108 patients with chronic urticaria treated in Ji′an County People′s Hospital from April 2022 to June 2023 were selected and divided into control group (n=58) and observation group (n=50) by random number table method. The control group was treated with loratadine, and the observation group was treated with loratadine combined with Yupingfeng granules. The effective rate, recurrence rate, serum inflammatory factor levels [interleukin (IL)-4, immunoglobulin E (IgE), IL-8, interferon-γ (INF-γ) and IL-17], symptom improvement degree [maximum diameter score of wheal, number score of wheal, daily wheal attack frequency score, itching degree score and wheal duration score], and cellular immune changes [CD4+, CD8+, CD4+/CD8+] were observed in the two groups Results The effective rate of treatment in the observation group was higher than that in the control group, and the recurrence rate was lower than that in the control group (P<0.05). After treatment, the levels of serum IL-4, IgE, IL-8 and IL-17 in the two groups were lower than those before treatment, the level of INF-γ was higher than that before treatment, and the levels of serum IL-4, IgE, IL-8 and IL-17 in the observation group were lower than those in the control group, the level of serum INF-γ was higher than that in the control group (P<0.05). After treatment, the scores of maximum diameter of wheal, number of wheal, number of daily wheal attacks, pruritus degree and duration of wheal in the two groups were lower than those before treatment, and the scores of maximum diameter of wheal, number of wheal, number of daily wheal attacks, pruritus degree and duration of wheal in the observation group were lower than those in the control group (P<0.05). After treatment, the levels of CD8+ in the two groups was lower than those before treatment, level of CD4+ and CD4+/CD8+ were higher than that before treatment, and the levels of CD8+ and in the observation group was lower than those in the control group, the level of CD4+ CD4+/CD8+ were higher than that in the control group (P<0.05).Conclusion Loratadine combined with Yupingfeng granules has a definite effect on chronic urticaria, which can improve the clinical symptoms and cellular immune function of patients, inhibit the inflammatory response of patients, and reduce the recurrence rate of patients. It is worthy of clinical application.
