摘要:
目的 系统评估蒲公英治疗急性乳腺炎的有效性与安全性。方法 全面检索中国知网、万方、维普、中国生物医学数据库、PubMed、Embase、Cochrane Library和Web of Science，检索时间为数据库建库至2023年4月25日，纳入研究蒲公英治疗急性乳腺炎的随机对照试验。由2位研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险，利用R软件进行Meta分析。结果 纳入6项研究，共524例急性乳腺炎患者；纳入研究质量偏低。Meta分析结果显示，干预组在治愈率[OR=4.31，95%CI（2.62，7.09），P=0.02]、总有效率[OR=5.07，95%CI（2.41，10.64），P=0.001]、缓解急性乳腺炎疼痛[MD=-0.75，95%CI（-1.12，-0.38），P=0.042]和改善炎症指标[MD=-1.72，95%CI（-2.63，-0.82），P=0.011]上优于对照组，在回乳情况上与对照组比较，差异无统计学意义[OR=1.17，95%CI（0.43，3.17），P=0.72]。两组均无严重不良事件报告。结论 蒲公英用于急性乳腺炎的治疗，可有效改善炎症和缓解患者的乳房疼痛，且安全性高。因纳入研究质量偏低，需高质量研究来探索蒲公英治疗急性乳腺炎的疗效。
Abstract:
Objective To systematically evaluate the efficacy and safety of dandelion in the treatment of acute mastitis. Methods CNKI, Wanfang, VIP, Chinese Biomedical Database, PubMed, Embase, Cochrane Library and Web of Science were searched from the establishment of the database to April 25, 2023. Randomized controlled trials of dandelion in the treatment of acute mastitis were included. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Meta-analysis was performed using R software. Results Six studies involving 524 patients with acute mastitis were included. The quality of included studies was low. The results of Meta-analysis showed that the intervention group was superior to the control group in the cure rate [OR=4.31, 95%CI(2.62, 7.09), P=0.02], total effective rate [OR=5.07, 95%CI(2.41, 10.64), P=0.001], relief of acute mastitis pain [MD=-0.75, 95%CI(-1.12, -0.38), P=0.042] and improvement of inflammatory indicators [MD=-1.72, 95%CI(-2.63, -0.82), P=0.011]. There was no significant difference in milk regurgitation between the two groups [OR=1.17, 95%CI(0.43, 3.17), P=0.72]. No serious adverse events were reported in both groups. Conclusion Dandelion used in the treatment of acute mastitis can effectively improve inflammation and relieve breast pain in patients with high safety. Due to the low quality of the included studies, high-quality studies are needed to explore the efficacy of dandelion in the treatment of acute mastitis.
