摘要:
目的 研究DRG实施后对三级老年病医院运营管理的影响,提出对策建议,为医院及科室精细化管理提供理论依据,促进医院高质量发展。方法 以三级老年病综合医院2022年7月1日-12月31日全部出院患者共计1512例作为研究对象,采用描述性统计学方法和文献查阅法,运用服务能力、服务效率和费用评价指标分析DRG开展后该院运营情况。结果 该院治疗同类疾病所需费用及例均住院时间高于全市(地区)的平均水平;有5个科室的CMI值均高于全院平均水平,科室占比为28.00%;在平均住院日中,康复医学科住院天数最长,为19.15 d,儿科最短,为5.26 d;在次均盈亏中,骨科盈利最多,为4501.99元,重症医学科亏损最多,为8400.01元;该院诊疗占比、药品占比、材料占比分别为75.86%、16.32%、7.82%。结论 通过调整临床路径,规范医疗行为,优化病种结构,提升医疗服务能力,积极探索发展模式,能够更好地适应医保支付方式改革带来的冲击,可有效提升运营效率,促进医院精细化管理。
Abstract:
Objective To study the impact of DRG implementation on the operation and management of tertiary geriatric hospitals, put forward countermeasures and suggestions, provide theoretical basis for the refined management of hospitals and departments, and promote the high-quality development of hospitals. Methods A total of 1512 discharged patients from July 1, to December 31, 2022 in a tertiary geriatric general hospital were selected as the research objects. Descriptive statistical methods and literature review methods were used to analyze the operation of the hospital after DRG was carried out by using service capacity, service efficiency and cost evaluation indicators. Results The cost and average hospitalization time of the hospital for the treatment of similar diseases were higher than the average level of the whole city (region). The CMI values of 5 departments were higher than the average level of the whole hospital, and the proportion of departments was 28.00%. In the average length of stay, the length of stay in the Department of Rehabilitation Medicine was the longest (19.15 days), and the shortest in the Department of Pediatrics (5.26 days). In the average profit and loss, the Department of Orthopedics had the most profit, which was 4501.99 yuan, and the Intensive Care Unit had the most loss, which was 8400.01 yuan. The proportion of diagnosis and treatment, the proportion of drugs and the proportion of materials in the hospital were 75.86%, 16.32% and 7.82%, respectively. Conclusion By adjusting the clinical pathway, standardizing medical behavior, optimizing the disease structure, improving the medical service ability, and actively exploring the development model, it can better adapt to the impact of the reform of medical insurance payment methods, effectively improve the operation efficiency, and promote the fine management of the hospital.
