摘要:
目的 探讨异丙酚复合地佐辛预防性镇痛对腹腔镜下胆囊切除术患者应激反应及术后疼痛的影响。方法 选取2021年1月-2023年11月德安县中医院收治的行腹腔镜胆囊切除术的患者198例作为研究对象,按照随机数字表法将所有患者分为对照组和研究组,每组99例。对照组采用常规麻醉,研究组采用异丙酚复合地佐辛麻醉。比较两组患者氧化应激指标[过氧化氢(H2O2)、丙二醛(MDA)及总抗氧化态(TAS)]水平、术后疼痛评分、血流动力学指标[收缩压(SBP)、舒张压(DBP)、心率(HR)]水平、不良反应(体动、呼吸抑制、恶心、呕吐、头痛)发生率。结果 研究组患者手术后H2O2、MDA水平低于对照组,TAS水平高于对照组(P<0.05)。研究组术后30 min、2 h及6 h的疼痛评分均低于对照组(P<0.05)。研究组T1、T2时HR、SBP、DBP高于对照组,T3时低于对照组(P<0.05);对照组T1、T2时HR、SBP、DBP低于T0时,T3时高于T0时,而研究组T1、T2时HR、SBP、DBP低于T0时(P<0.05)。研究组T3时HR、SBP、DBP与T0时比较,差异无统计学意义(P>0.05)。研究组患者不良反应发生率低于对照组(P<0.05)。结论 异丙酚复合地佐辛能够有效改善腹腔镜下胆囊切除术患者术后疼痛情况,患者不良反应少,氧化应激指标及血流动力学稳定性高,值得临床应用。
Abstract:
Objective To investigate the effect of preventive analgesia with propofol combined with dezocine on stress response and postoperative pain in patients undergoing laparoscopic cholecystectomy. Methods A total of 198 patients who underwent laparoscopic cholecystectomy in De’an County Hospital of Traditional Chinese Medicine from January 2021 to November 2023 were selected as the research objects. According to the random number table method, all patients were divided into control group and study group, with 99 patients in each group. The control group was treated with routine anesthesia, and the study group was treated with propofol combined with dezocine anesthesia. The levels of oxidative stress indexes [hydrogen peroxide (H2O2), malondialdehyde (MDA) and total antioxidant state (TAS)], postoperative pain score, hemodynamic indexes [systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR)] and the incidence of adverse reactions (body movement, respiratory depression, nausea, vomiting, headache) were compared between the two groups. Results The levels of H2O2 and MDA in the study group were lower than those in the control group, and the level of TAS was higher than that in the control group (P<0.05). The pain score of the study group at 30 min, 2 h and 6 h after operation was lower than that of the control group (P<0.05). HR, SBP and DBP in the study group were higher than those in the control group at T1 and T2, and lower than those in the control group at T3 (P<0.05). In the control group, HR, SBP and DBP at T1 and T2 were lower than those at T0, and those at T3 were higher than those at T0, while in the study group, HR, SBP and DBP at T1 and T2 were lower than those at T0 (P<0.05). There was no significant difference in HR, SBP and DBP between T3 and T0 in the study group (P>0.05). The incidence of adverse reactions in the study group was lower than that in the control group (P<0.05). Conclusion Propofol combined with dezocine can effectively improve the postoperative pain of patients undergoing laparoscopic cholecystectomy, with less adverse reactions, high oxidative stress index and hemodynamic stability, which is worthy of clinical application.
