摘要:
目的 分析成人乙型肝炎疫苗全程接种的影响因素及免疫效果。方法 选取峡江县2023年1月-12月接种乙肝疫苗的成人180例作为研究对象，根据是否全程接种乙肝疫苗分为研究组（n=85）和对照组（n=95）。对照组患者到期未接种，医护人员连续联系＞3次仍未接种。研究组患者按照0-1-6月免疫程序进行乙肝疫苗接种，在规定日期内连续完成3次接种。采用多因素Logisitic回归分析乙肝疫苗全程接种的影响因素，比较两组乙肝表面抗原（HBsAg）阳性率及乙肝表面抗体（HBsAb）阳性率。结果 单因素分析显示，性别及婚姻状况对成人乙型肝炎疫苗全程接种影响不大（P＞0.05）；而年龄、文化程度、乙肝家族史、乙肝防护行为、乙肝知识认知情况及职业等均与成人乙型肝炎疫苗全程接种间存在较大影响（P＞0.05）。多因素Logistic回归分析表明，年龄、文化程度、乙肝家族史、职业、乙肝知识认知情况均为成人乙肝疫苗全程接种的独立影响因素（P＜0.05）。研究组HBsAg阳性率低于对照组，HBsAb阳性率高于对照组，差异有统计学意义（P＜0.05）。结论 成人乙肝疫苗全程接种的相关影响因素较多，有必要加强健康宣教，提高疫苗全程接种率及免疫效果，降低乙肝病毒感染风险。
Abstract:
Objective To analyze the influencing factors and immune effects of whole course vaccination of hepatitis B vaccine in adults. Methods A total of 180 adults who were vaccinated with hepatitis B vaccine from January to December 2023 in Xiajiang County Center for Disease Control and Prevention were selected as the research objects. According to whether the whole course of hepatitis B vaccine was vaccinated, they were divided into study group (n=85) and control group (n=95). The patients in the control group did not receive vaccination at the expiration date, and the medical staff contacted them for more than 3 times and still did not receive vaccination. Patients in the study group were vaccinated with hepatitis B vaccine according to the 0-1-6 month immunization program, and three vaccinations were completed continuously within the specified date. Multivariate Logisitic regression analysis was used to analyze the influencing factors of hepatitis B vaccination. The positive rates of hepatitis B surface antigen (HBsAg) and hepatitis B surface antibody (HBsAb) were compared between the two groups. Results Univariate analysis showed that gender and marital status had little effect on adult hepatitis B vaccination (P>0.05), while age, education level, family history of hepatitis B, hepatitis B protection behavior, hepatitis B knowledge cognition and occupation had great influence on the whole course vaccination of hepatitis B vaccination in adults (P>0.05). Multivariate Logistic regression analysis showed that age, education level, family history of hepatitis B, occupation, and knowledge of hepatitis B were independent influencing factors for the whole course vaccination of hepatitis B vaccination in adults (P<0.05). The positive rate of HBsAg in the study group was lower than that in the control group, and the positive rate of HBsAb was higher than that in the control group, the differences were statistically significant (P<0.05). Conclusion There are many related factors affecting the whole course vaccination of hepatitis B vaccine in adults. It is necessary to strengthen health education, improve the whole course vaccination rate and immune effect, and reduce the risk of hepatitis B virus infection.
