摘要:
目的 基于系统评价研究羟苯磺酸钙对ACEI治疗早期糖尿病肾病增效作用的规律。方法 检索中国知网（CNKI）、万方（Wanfang Data）、维普（VIP）、中国生物医学 （CBM）、PubMed、EMbase以及Cochrane Library等数据库中自建库至2024年2月的临床随机对照试验（RCTs），筛选符合标准的文献，提取并评估数据，采用RevMan 5.4软件和Excel软件进行评价。结果 最终纳入11项研究，共1410例患者。Meta分析结果显示，试验组Δ尿微量白蛋白（ΔmA1b）（P=0.0006），Δ尿白蛋白排泄率（ΔUAER）（P=0.02）大于对照组。亚组分析结果显示，在降低尿微量白蛋白上，羟苯磺酸钙对贝那普利和依那普利均有增效作用，且对两者20 mg剂量组的增效作用优于10 mg剂量组，对于贝那普利20 mg剂量组增效作用优于依那普利20 mg剂量组（P＜0.01），但两者10 mg剂量组的增效作用相当。在降低尿白蛋白排泄率上，羟苯磺酸钙对赖诺普利、培哚普利、贝那普利均有增效作用，其中对贝那普利10 mg剂量组与赖诺普利10 mg剂量组增效作用相当，且均优于培哚普利4 mg剂量组（P＜0.01）。结论 羟苯磺酸钙对ACEI治疗早期糖尿病肾病有一定的增效作用，且因药物选择、剂量的不同，呈现不同的增效结果。
Abstract:
Objective To study the law of synergistic effect of calcium dobesilate combined with acei in the treatment of early diabetic nephropathy based on the systematic evaluation. Methods Randomized controlled trials (RCTs) were searched in the CNKI, Wanfang Data, VIP CBM, PubMed, Embase and Cochrane Library databases inception to February 2024. The literatures that met the criteria were screened, the data were extracted and evaluated. Then systematic evaluation was performed by Revman 5.4 software. Results Eleven studies with 1410 patients were included. The results of meta-analysis showed that The Δurinary microalbumin (ΔmAlb) (P=0.0006) and Δurinary albumin excretion rate (ΔUAER) (P=0.02) in the experimental group were higher than those in the control group. The results of subgroup analysis showed in terms of reducing urinary microalbumin, calcium dobesilate had synergistic effect on benazepril and enalapril, the synergistic effect of 20 mg dose group was significantly better than that of 10 mg dose group (P<0.01), and the synergistic effect of benazepril 20 mg dose group was also significantly better than that of enalapril 20 mg dose group (P<0.01), but the synergistic effect of the two 10 mg dose groups was similar; in reducing urinary albumin excretion rate, calcium dobesilate had synergistic effect on lisinopril, perindopril and benazepril, while the synergistic effect of benazepril 10mg dose group was similar to that of lisinopril 10 mg dose group, and both were better than perindopril 4 mg dose group (P<0.01). Conclusion Calcium dobesilate has a certain synergistic effect on ACEI in the treatment of early diabetic nephropathy, and there are different synergistic results due to the different drug selection and dosage.
