摘要:
目的 研究泌尿系结石合并感染患者临床病原菌检测结果。方法 选取2021年1月-2023年12月我院诊治的321例泌尿系结石合并感染患者为研究对象,均进行中段尿培养,观察病原菌检测结果,分析泌尿系结石合并感染病原菌种类、不同临床特征患者病原菌种类以及主要病原菌耐药性。结果 321例泌尿系结石合并感染患者共培养出321株,病原菌以革兰阴性杆菌为主,检出193株,占60.12%;2021-2023年期间病原菌检出率比较,差异无统计学意义(P>0.05)。不同直径、部位泌尿系结石合并感染患者病原菌种类比较,差异无统计学意义(P>0.05),而复发泌尿系结石合并感染患者病原菌中革兰阴性菌占比高于无泌尿系结石合并感染患者(P<0.05)。大肠埃希菌、铜绿假单胞菌、肺炎克雷伯菌对莫西沙星、环丙沙星、头孢唑林、头孢呋辛、哌拉西林、氨苄西林、替卡西林、复方磺胺甲唑的耐药率高于亚胺培南、阿莫西林-克拉维酸钾、阿莫西林(P<0.05);粪肠球菌、屎肠球菌对苯唑西林、青霉素、氨苄西林、庆大霉素的耐药率高于万古霉素、左氧氟沙星、莫西沙星(P<0.05)。结论 泌尿系结石合并感染患者病原菌分布具有一定的特点,主要以革兰阴性菌为主,临床应避免经验性用药,依据病原菌检测和药敏试验结果科学合理选择药物进行有效治疗。
Abstract:
Objective To study the detection results of clinical pathogens in patients with urinary calculi complicated with infection. Methods A total of 321 patients with urinary calculi complicated with infection diagnosed and treated in our hospital from January 2021 to December 2023 were selected as the research objects. All patients underwent midstream urine culture. The results of pathogen detection were observed. The types of pathogenic bacteria in urinary calculi complicated with infection, the types of pathogenic bacteria in patients with different clinical characteristics and the drug resistance of main pathogenic bacteria were analyzed. Results Totally 321 strains were cultured from 321 patients with urinary calculi complicated with infection, gram-negative bacilli were the main pathogens (193 strains), accounting for 60.12%. There was no significant difference in the detection rate of pathogenic bacteria during 2021-2023 (P>0.05). There was no significant difference in the types of pathogenic bacteria in patients with different diameters and sites of urinary calculi complicated with infection (P>0.05), while the proportion of gram-negative bacteria in patients with recurrent urinary calculi complicated with infection was higher than that in patients without urinary calculi complicated with infection (P<0.05). The resistance rates of Escherichia coli, Pseudomonas aeruginosa and Klebsiella pneumoniae to moxifloxacin, ciprofloxacin, cefazolin, cefuroxime, piperacillin, ampicillin, ticarcillin and compound sulfamethoxazole were higher than those of imipenem, amoxicillin-clavulanate potassium and amoxicillin (P<0.05). The resistance rates of Enterococcus faecalis and Enterococcus faecium to oxacillin, penicillin, ampicillin and gentamicin were higher than those of vancomycin, levofloxacin and moxifloxacin (P<0.05). Conclusion The distribution of pathogenic bacteria in patients with urinary calculi complicated with infection has certain characteristics, mainly gram-negative bacteria. Clinically, empirical medication should be avoided, and drugs should be selected scientifically and reasonably according to the results of pathogenic bacteria detection and drug sensitivity test for effective treatment.
