摘要:
目的 探究呼吸功能训练结合综合护理干预对慢性阻塞性肺气肿患者心理状态及肺功能的影响。方法 以随机数字表法将2021年1月-2023年11月乐安县人民医院收治的52例慢性阻塞性肺气肿患者分为对照组和研究组，每组26例。对照组采用常规护理，研究组采取呼吸功能训练结合综合护理。比较两组心理状态（SAS评分、SDS评分）、肺功能指标[第1秒用力呼气容积占预计值百分比（FEV1）、用力肺活量（FVC）、一秒率（FEV1/FVC）、最大呼气中段流速（MMEF）、呼气峰值流速（PEF）]、生活质量、护理满意度、并发症（自发性气胸、慢性肺源性心脏病、呼吸衰竭）发生率。结果 在SAS、SDS评分方面，两组护理后均改善，其中研究组改善程度优于对照组（P＜0.05）；在肺功能方面，两组护理后FVC、FEV1、FEV1/FVC、MMEF及PEF均较护理前提升，且研究组各指标改善程度均优于对照组（P＜0.05）；在生活质量方面，两组护理后生活质量量表各维度评分均提高，且研究组各维度评分均高于对照组（P＜0.05）；研究组护理满意度较对照组更高，并发症发生率较对照组更低（P＜0.05）。结论 呼吸功能训练结合综合护理干预在改善慢性阻塞性肺气肿患者心理状态和肺功能、提高患者生活质量和护理满意度、降低并发症发生率方面具有积极作用。
Abstract:
Objective To explore the effect of respiratory function training combined with comprehensive nursing intervention on mental status and pulmonary function in patients with chronic obstructive emphysema. Methods Fifty-two patients with chronic obstructive pulmonary emphysema admitted to Le′an County People′s Hospital from January 2021 to November 2023 were enrolled and randomly assigned via a random number table to either a control group or a study group, with 26 patients in each group. The control group was given routine nursing, while the study group was given respiratory function training combined with comprehensive nursing. The mental status (SAS score, SDS score), pulmonary function indexes [percentage of forced expiratory volume in the first second to predicted value (FEV1), forced vital capacity (FVC), forced expiratory volume in one second/forced vital capacity (FEV1/FVC), maximum midexpiratory flow rate (MMEF), peak expiratory flow rate (PEF)], quality of life, nursing satisfaction, complications (spontaneous pneumothorax, chronic pulmonary heart disease, respiratory failure) were compared between the two groups. Results In terms of SAS and SDS scores, both groups improved after nursing, and the improvement of the study group was better than that of the control group (P<0.05). In terms of pulmonary function, FVC, FEV1, FEV1/FVC, MMEF and PEF in the two groups after nursing were higher than those before nursing, and the improvement of each index in the study group was better than that in the control group (P<0.05). In terms of quality of life, the scores of each dimension of the quality of life scale of the two groups after nursing were improved, and the scores of each dimension of the study group were higher than those of the control group (P<0.05). The nursing satisfaction of the study group was higher than that of the control group, and the incidence of complications was lower than that of the control group (P<0.05). Conclusion Respiratory function training combined with comprehensive nursing intervention has a positive effect on improving the mental state and pulmonary function of patients with chronic obstructive pulmonary emphysema, improving their quality of life and nursing satisfaction, and reducing the incidence of complications.
