摘要:
目的 通过分析案例医院DRG高倍率与正常倍率病例之间的差异，探讨影响高倍率病例住院费用的主要因素。方法 对案例医院2023年出院患者信息及费用进行描述，住院费用进行结构和灰色关联分析。结果 高倍率与正常倍率病例在住院天数、性别、住院费用方面存在显著差异，高倍率病例住院费用占比前3分别为药费（32.42%）、诊断费（21.24%）、治疗费（16.24%），正常倍率病例为诊断费（28.55%）、药费（20.38%）、材料费（19.21%）；高倍率住院费用关联度前3个分别为手术费（0.872）、诊断费（0.787）、药费（0.778），正常倍率组为材料费（0.96）、诊断费（0.945）、一般医疗费（0.942）。结论 诊断费、药费和手术费是影响高倍率病例费用的主要因素，应调整费用结构，加强成本控制，提高检查检验质量，规范药物使用管理，开展多学科诊疗，提升医疗质量，从而控制高倍率病例的住院费用。
Abstract:
Objective To explore the main factors affecting the hospitalization costs of high magnification cases by analyzing the difference between high magnification and normal magnification cases of DRG in case hospitals. Methods The information and costs of discharged patients in the case hospital in 2023 were described, and the hospitalization costs were analyzed by structure and grey correlation analysis. Results There were significant differences in hospitalization days, gender and hospitalization costs between high magnification cases and normal magnification cases. The top three hospitalization costs of high magnification cases were drug costs (32.42%), diagnosis costs (21.24%) and treatment costs (16.24%), while those of normal magnification cases were diagnosis costs (28.55%), drug costs (20.38%) and material costs (19.21%). The top three of the correlation degree of hospitalization costs at high magnification were operation costs (0.872), diagnosis costs (0.787) and drug costs (0.778), while those of normal magnification group were material costs (0.96), diagnosis costs (0.945) and general medical costs (0.942). Conclusion Diagnosis costs, drug costs and operation costs are the main factors affecting the cost of high magnification cases. It is necessary to adjust the cost structure, strengthen cost control, improve the quality of examination and inspection, standardize the management of drug use, carry out multidisciplinary diagnosis and treatment, and improve the quality of medical treatment, so as to control the hospitalization cost of high magnification cases.
