摘要:
目的 回顾性分析某三级甲等医院2018-2023年恶性肿瘤住院患者病历资料，探讨疾病构成、变化趋势及人口统计学特征，为本地区恶性肿瘤防治策略制定提供依据。方法 收集某院 2018-2023年HIS系统病案首页数据，统计肿瘤患者的性别、年龄、癌种等的分布情况。结果 共纳入10 000例恶性肿瘤患者，其中男女比例为1.02∶1。恶性肿瘤患者主要为50岁以上人群，60~70岁及70岁以上高龄男性患者多于女性，其余年龄组女性占比较高，且乳腺癌患者人数居首位。随年龄增长，乳腺癌、肺癌等发病率存在一定变化。不同血糖情况的乳腺癌患者在合并高血压、组织学分级等方面比较，差异有统计学意义（P＜0.05），各组内ER阴性患者多呈现不良病理特征。结论 该地区中老年为恶性肿瘤高发人群，不同性别、年龄肿瘤发病各有特点，糖尿病患者中乳腺癌发病率可能较高且预后差，建议对乳腺癌患者常规行OGTT检测，以改善治疗与预后。
Abstract:
Objective To retrospectively analyze the medical records of inpatients with malignant tumors in a tertiary hospital from 2018 to 2023, and to explore the disease composition, change trend and demographic characteristics, so as to provide a basis for the formulation of malignant tumor prevention and control strategies in this region. Methods The data on the front pages of medical records in the HIS system of a certain hospital from 2018 to 2023 were collected, and the distribution of gender, age, cancer types and other aspects of tumor patients was counted. Results The male-to-female ratio among 10 000 cases of malignant tumors was 1.02∶1. The majority of patients with malignant tumors were over 50 years old. Among the elderly, the number of male patients aged 60-70 and over 70 was higher than that of female patients. In other age groups, the proportion of female patients was relatively high. The number of breast cancer patients was the highest. With age, the incidence rates of breast cancer and lung cancer showed certain changes. There were significant differences in hypertension and histological grade among breast cancer patients with different blood glucose levels (P<0.05). ER-negative patients in each group showed poor pathological features. Conclusion Middle-aged and elderly people are the high-risk groups for malignant tumors. The occurrence of tumors has its characteristics among different genders and ages. The incidence of breast cancer in diabetic patients may be relatively high and the prognosis may be poor. It is recommended that OGTT test should be routinely carried out for breast cancer patients to improve their treatment and prognosis.
