摘要:
目的 研究经颅多普勒超声（TCD）联合颈动脉超声（CDUS）对老年脑梗死（CI）患者血管病变的评估价值。方法 纳入2022年9月-2024年9月金溪县中医院收治的60例老年CI患者，行TCD、CDUS及联合诊断，统计TCD、CDUS及联合检测的阳性检出率，分析其对老年CI患者血管病变的诊断效能，比较TCD、CDUS及其联合检测在不同类型CI患者中的阳性诊断准确率。结果 TCD联合CDUS检测的阳性检出率高于二者单一检测（P＜0.05）。TCD联合CDUS检测对老年CI患者血管病变的诊断准确性、敏感度、特异度高于其单独检测（P＜0.05）。TCD联合CDUS检测在大面积脑梗死、中面积脑梗死患者中的阳性诊断准确率高于其单一检测（P＜0.05）。结论 TCD联合CDUS对老年CI患者的血管病变情况具有确切评估价值，其诊断效能高，可提高大面积脑梗死、中面积脑梗死患者的阳性诊断准确性率。
Abstract:
Objective To study the value of transcranial Doppler ultrasound (TCD) combined with carotid duplex ultrasonography (CDUS) in the evaluation of vascular lesions in elderly patients with cerebral infarction (CI). Methods Sixty elderly patients with CI admitted to Jinxi County Hospital of Traditional Chinese Medicine from September 2022 to September 2024 were included. TCD, CDUS and combined diagnosis were performed. The positive detection rates of TCD, CDUS and combined detection were counted, and their diagnostic efficacy for vascular lesions in elderly CI patients was analyzed. The positive diagnostic accuracy of TCD, CDUS and their combined detection in different types of CI patients was compared. Results The positive detection rate of TCD combined with CDUS was higher than that of single detection (P<0.05). The diagnostic accuracy, sensitivity and specificity of TCD combined with CDUS in the diagnosis of vascular lesions in elderly CI patients were higher than those of single detection (P<0.05). The positive diagnostic accuracy of TCD combined with CDUS detection in patients with large area cerebral infarction and medium area cerebral infarction was higher than that of single detection (P<0.05). Conclusion TCD combined with CDUS has a definite evaluation value for vascular lesions in elderly patients with CI, and its diagnostic efficiency is high, which can improve the positive diagnostic accuracy rate of patients with large area cerebral infarction and medium area cerebral infarction.
