摘要:
目的 观察CT值变化对少量渗血出血性脑梗死的诊断价值。方法 选取2016年10月-2024年7月天津市南开区中医医院收治的疑似脑梗死患者110例作为研究对象,患者分别实施MRI检查与CT检查,将MRI结果作为诊断“金标准”,将患者划分为研究组77例、对照组33例,研究组为少量渗血出血性脑梗死患者,对照组为单纯缺血性脑梗死患者。比较两组ΔCT值,并采用ROC曲线分析ΔCT值诊断少量渗血出血性脑梗死患者的效能。结果 研究组ΔCT值为(18.92±7.96)Hu,高于对照组的(6.58±1.06)Hu(P<0.05);基于ROC曲线的分析,ΔCT值被确定为12.5 Hu时,其诊断效能最佳,敏感度可达93.75%,特异度达91.67%。结论 测定CT值变化在少量渗血出血性脑梗死患者中的诊断价值确切。当疑似少量渗血区域与同期梗死区域的ΔCT值达到或超过12.5 Hu时,表明缺血性脑梗死区域存在少量渗血的可能性较高;而当ΔCT值低于12.5 Hu时,则表明缺血性梗死区域内少量渗血的可能性较低。
Abstract:
Objective To observe the diagnostic value of CT value changes in a small amount of bleeding hemorrhagic cerebral infarction. Methods A total of 110 patients with suspected cerebral infarction admitted to Tianjin Nankai District Hospital of Traditional Chinese Medicine from October 2016 to July 2024 were selected as the research objects. The patients were examined by MRI and CT respectively. The MRI results were used as the "gold standard" for diagnosis. The patients were divided into study group (77 patients) and control group (33 patients). The study group was a small amount of patients with hemorrhagic cerebral infarction, and the control group was patients with simple ischemic cerebral infarction. The ΔCT values of the two groups were compared, and the ROC curve was used to analyze the efficacy of ΔCT value in the diagnosis of patients with a small amount of hemorrhagic cerebral infarction. Results The ΔCT value of the study group was (18.92±7.96) Hu, which was higher than that of the control group (6.58±1.06) Hu (P<0.05). Based on the analysis of ROC curve, when the ΔCT value was determined to be 12.5 Hu, the diagnostic efficiency was the best, with a sensitivity of 93.75% and a specificity of 91.67%. Conclusion The diagnostic value of CT value changes in patients with a small amount of bleeding hemorrhagic cerebral infarction is accurate. When the ΔCT value of the suspected small amount of bleeding area and the infarct area in the same period reaches or exceeds 12.5 Hu, it indicates that there is a high possibility of a small amount of bleeding in the ischemic cerebral infarction area; when the ΔCT value is lower than 12.5 Hu, it indicates that the possibility of a small amount of bleeding in the ischemic infarct area is low.
