摘要:
目的分析腰麻前预灌充胶体对产妇及新生儿心肌酶和肌钙蛋白的影响遥方法选取本院80 例剖宫产产妇随机分为预 扩容组（n=40，采用胶体预扩容）和对照组（n=40，未用胶体预扩容）遥记录两组新生儿脐动脉血中的LDH尧CK-MB尧CK尧AST尧 CTN和监测麻醉前（T1）尧麻醉后1 min（T2）尧麻醉后5 min（T3）尧麻醉后10 min（T4）尧胎儿娩出（T5）尧胎儿娩出后10 min（T6）产妇 的心率尧舒张压尧收缩压遥结果预扩容组的LDH（281.5依32.3）U/L低于对照组的LDH（503.4依73.4）U/L，预扩容组的CK-MB （26.7依8.2）U/L 低于对照组的CK-MB（56.3依20.6）U/L，预扩容组的CK（230.4依45.3）U/L 低于照组的CK（571依74.3），预扩容 组的AST（36.9依7.6）U/L 低于对照组的AST（57.8依19.8）U/L，差异具有统计学意义（约0.05）曰预扩容组与对照组的心率尧血氧 饱和度尧呼吸比较差异无统计学意义（跃0.05），预扩容组手术开始与手术中的收缩压以及舒张压高于对照组，比较差异具有统 计学意义（约0.05）遥结论术前静脉输注胶体扩容液能够维持产妇血流动力学的稳定更有优势，保证母体对胎儿的血流灌注， 对新生儿缺血缺氧导致的宫内窘迫具有预防作用，值得在临床上推广使用遥
Abstract:
Abstract：Objective To analyze the effects of pre-anesthesia with spinal anesthesia on myocardial enzymes and troponin in maternal and newborn infants.Methods Select hospital were randomly divided into 80 cases of pre-expansion cesarean section group (n=40, using the pre-expansion colloid) and control group (n=40,not adopt the pre-expansion with colloidal).The levels of LDH,CK-MB,CK, AST and CTN in umbilical arterial blood of two newborns were recorded before anesthesia (T1),1 min after anesthesia (T2),5 min after anesthesia(T3),10 min after anesthesia (T4),fetal delivery(T5),10 min after the delivery of the fetus(T6)maternal heart rate,diastolic blood pressure,systolic blood pressure.Results LDH (281.5依32.3)U/L in the pre-dilatation group was lower than that in the control group (503.4依73.4)U/L,The CK-MB(26.7依8.2)U/L in the pre-dilatation group was lower than that in the control group(56.3依20.6)U/L,In predilatation group,CK (230.4依45.3)U/L was lower than CK(571依74.3)U/L,AST(36.9依7.6)U/L in pre-dilatation group was lower than that in control group (57.8依19.8)U/L,the difference was statistically significant ( <0.05).There was no significant difference in heart rate, oxygen saturation and respiration between the pre-dilatation and control groups ( >0.05),Start the pre expansion surgery group with intraoperative systolic blood pressure and diastolic blood pressure was higher than the control group,the difference was statistically significant ( <0.05).Conclusion Preoperative intravenous injection of colloidal liquid expansion has more advantages to maintain the maternal hemodynamic stability,ensure perfusion on fetal maternal, has preventive effects lead to neonatal hypoxic ischemia the fetal distress,worthy of promotion in clinical use
