摘要:
目的 测定2型糖尿病合并抑郁症患者血清25-羟维生素D3水平,探讨维生素D缺乏与2型糖尿病合并抑郁症的关系。方法 选择2015年9月~2017年3月于我院住院的2型糖尿病患者84例,根据HAMD评分将患者分为T2DM组44例和T2DMD组40例。收集两组患者临床资料,检测FBG、HbA1c、TC、TG、LDL-C、HDL-C、血清25-羟维生素D3水平,进行Pearson相关分析和Logistic回归分析,并比较两组患者血清25-羟维生素D3缺乏患病率。结果 两组患者年龄、BMI、TC、TG比较,差异无统计学意义(P>0.05);T2DMD组FBG、HbA1c、LDL-C均高于T2DM组,且糖尿病病程长于T2DM组,差异具有统计学意义(P<0.05)。T2DM组血清25-羟维生素D3平均水平为(20.94±1.68)ng/ml,高于T2DMD组的(16.44±2.05)ng/ml,差异具有统计学意义(P<0.05)。T2DMD组血清25-羟维生素D3缺乏患病率为95.00%,高于T2DM组的31.82%,差异具有统计学意义(P<0.05)。Pearson相关分析显示,血清25-羟维生素D3水平与FBG、HbA1c、LDL-C、病程均呈负相关(P<0.05),与HDL-C呈正相关(P<0.05),与BMI、TC及TG均无相关性(P>0.05)。多元Logistic回归分析显示,血清25-羟维生素D3水平、FBG、HbA1c及病程是2型糖尿病合并抑郁症的独立危险因素(P<0.05)。结论 2型糖尿病合并抑郁症患者维生素D缺乏更加普遍,维生素D缺乏可能参与2型糖尿病合并抑郁症的发病过程。
Abstract:
Objective To determine the serum level of 25-hydroxyvitamin D3 in patients with type 2 diabetes mellitus complicated with depression,and to explore the relationship between vitamin D deficiency and type 2 diabetes mellitus with depression.Methods A total of 84 patients with type 2 diabetes mellitus admitted to our hospital from September 2015 to March 2017 were enrolled.Patients were divided into T2DM group(44 cases)and T2DMD group(40 cases)according to the HAMD score.The clinical data of the two groups were collected to detect FBG,HbA1c,TC,TG, LDL-C,HDL-C,serum 25-hydroxyvitamin D3 levels,Pearson correlation analysis and logistic regression analysis,the prevalence of serum 25-hydroxyvitamin D3 deficiency was compared between the two groups.Results There were no significant differences in age,BMI,TC and TG between the two groups(P>0.05).The FBG,HbA1c and LDL-C in the T2DMD group were higher than those in the T2DM group,and the duration of diabetes was longer than that in the T2DM group,the difference was statistically significant(P<0.05).The mean serum 25-hydroxyvitamin D3 level in the T2DM group was(20.94±1.68)ng/ml,which was higher than that in the T2DMD group(16.44±2.05)ng/ml,the difference was statistically significant(P<0.05).The prevalence of serum 25-hydroxyvitamin D3 deficiency in the T2DMD group was 95.00%,which was higher than that in the T2DM group 31.82%, the difference was statistically significant(P<0.05).Pearson correlation analysis showed that serum 25-hydroxyvitamin D3 levels were negatively correlated with FBG,HbA1c,LDL-C,and disease course(P<0.05),positively correlated with HDL-C(P<0.05),there was no correlation with BMI,TC and TG(P>0.05).Multivariate logistic regression analysis showed that serum 25-hydroxyvitamin D3 levels,FBG,HbA1c and disease duration were independent risk factors for type 2 diabetes mellitus with depression(P<0.05).Conclusion Vitamin D deficiency is more common in patients with type 2 diabetes mellitus and depression.Vitamin D deficiency may be involved in the pathogenesis of type 2 diabetes mellitus with depression.
