摘要:
目的 比较缩宫素与COOK双球囊促进宫颈成熟合并引产的临床效果。方法 选择2017年6月~2018年6月我院收治的108例足月引产孕妇临床资料，随机分为对照组和观察组，各54例，对照组采用缩宫素促进宫颈成熟并引产，观察组采用COOK双球囊促进宫颈成熟后再静滴缩宫素。比较两组孕妇宫颈成熟率、引产方式、引产后母婴相关并发症等。结果 观察组宫颈成熟率100.00%、阴道分娩率85.18%分别高于对照组的79.62%、72.22%，差异有统计学意义（P＜0.05）；观察组剖宫产率14.81%低于对照组27.78%，差异有统计学意义（P＜0.05）；观察组临产时间（18.20±6.23）h、总产程时间（6.89±2.15）h少于对照组（27.41±9.50）h、总产程时间（8.93±2.68）h，差异有统计学意义（P＜0.05）；观察组产后出血量、宫颈裂伤发生率、阴道血肿发生率、新生儿窒息发生率、Apgar评分与对照组对比，差异无统计学意义（P＞0.05）。结论 应用COOK双球囊促进宫颈成熟效果优于缩宫素，且未增加母婴引产后并发症，用于引产安全有效。
Abstract:
Objective To compare the clinical effects of oxytocin and COOK double balloon in promoting cervical ripening and induction of labor.Methods The clinical data of 108 full-term pregnant women admitted to our hospital from June 2017 to June 2018 were randomly divided into control group and observation group,54 cases in each group.The control group used oxytocin to promote cervical ripening and induce labor,the observation group used COOK double balloon to promote cervical ripening and then intravenous oxytocin.The cervical ripening rate,induction of labor,and maternal and child-related complications after induction of labor were compared between the two groups.Results The cervical ripening rate was 100.00% and the vaginal delivery rate was 85.18% in the observation group,which was higher than 79.62% and 72.22% in the control group, respectively,the difference was statistically significant(P<0.05).The cesarean section rate of the observation group was 14.81% lower than that of the control group 27.78.%,the difference was statistically significant(P<0.05);the observation group labor time(18.20±6.23)h, total labor time(6.89±2.15)h was less than the control group(27.41±9.50)h,total labor time (8.93±2.68)h,the difference was statistically significant(P<0.05);There were no significant differences in postpartum hemorrhage,incidence of cervical laceration,incidence of vaginal hematoma,incidence of neonatal asphyxia,and Apgar score in the observation group compared with the control group(P>0.05).Conclusion The application of COOK double balloon to promote cervical ripening is better than oxytocin,and it does not increase the complications of maternal and child induction,which is safe and effective for induction of labor.
