摘要:
目的 研究无创机械通气用于COPD合并呼吸衰竭患者的疗效。方法 我院2015年8月~2017 年4月收治COPD并发呼吸衰竭患者54例,随机分为观察组和对照组,每组27例。观察组进行常规治疗,对照组常规治疗基础上进行无创通气,两组患者如病情无明显改善或进步恶化时使用有创机械通气。记录患者的呼吸频率 、心率 、动脉血pH、二氧化碳分压 、氧分压变化、有创机械通气率及病死率变化。结果 治疗后观察组RR(18.2±5.1)次/min和HR(84.7±7.2)次/min、PaCO2(53.6±5.5)mmHg、PaO2(78.9±10.8)mmHg、pH(7.69±0.05)优于对照组(21.4±5.6)次/min、(96.2±6.2)次/min、(72.3±7.1)mmHg、(64.8±10.1)mmHg、(7.21±0.12),差异均存在统计学(P<0.05);病死率两组间比较无差异,不存在统计学意义(P>0.05)。结论 COPD呼吸衰竭患者加用无创通气可以改善呼吸功能和提高治愈率。
Abstract:
Abstract:Objective To study the curative effect of non-invasive mechanical ventilation on COPD patients with respiratory failure. Methods From August 2015 to April 2017,54 patients with COPD complicated by respiratory failure were randomly divided into observation group and control group,with 27 cases in each group.The observation group was treated routinely,and the control group was given non-invasive ventilation on the basis of routine treatment.Patients in both groups were given mechanical ventilation if their condition did not improve or worsen.The patients' respiratory frequency,heart rate,arterial blood pH,carbon dioxide partial pressure, oxygen partial pressure change,invasive mechanical ventilation rate and mortality rate were recorded.Results After treatment,the observation group RR(18.2±5.1)/min and HR(84.7±7.2)/min,PaCO2(53.6±5.5)mmHg,PaO2(78.9±10.8)mmHg,pH(7.69±0.05)better than the control group(21.4±5.6)/min,(96.2±6.2),/min(72.3±7.1),mmHg(64.8±10.1),mmHg(7.21±0.12),there were statistically significant differences(P<0.05);the mortality rate was no difference between the two groups was not statistically significant(P>0.05).Conclusion Noninvasive ventilation in patients with COPD respiratory failure can improve the respiratory function and improve the cure rate.
