摘要:
目的 探讨在腰麻-硬膜外联合阻滞中应用罗哌卡因、布比卡因的临床效果。方法 随机选取我院2016年2月~2017年12月收治的110例行剖宫产孕妇作为研究对象,采用奇偶分组的方式分为罗哌卡因组和布比卡因组,每组55例。罗哌卡因组采取罗哌卡因单次腰麻,布比卡因组采取布比卡因单次腰麻,对比两组产妇的、麻醉效果、不良反应发生情况。结果 罗哌卡因组的感觉阻滞起效时间、达到最高阻滞平面时间和运动阻滞起效时间分别为(73.01±14.13)s、(6.72±1.14)min和(2.64±0.63)min,均慢于布比卡因组的(59.10±8.06)s、(5.51±1.42)min和(2.12±0.44)min,差异具有统计学意义(P<0.05)。罗哌卡因组的运动完全恢复时间为(2.12±0.43)h,快于布比卡因组的(2.61±0.53)h,差异具有统计学意义(P<0.05)。布比卡因组的不良反应发生率为9.09%,高于罗哌卡因组的23.64%,差异具有统计学意义(P<0.05)。结论 在腰麻-硬膜外联合阻滞中,较之于布比卡因,应用罗哌卡因的临床效果更为确切,患者的运动完全恢复时间更短,不良反应发生率更低。
Abstract:
Objective To investigate the clinical effects of ropivacaine and bupivacaine in spinal anesthesia combined with epidural block. Methods A total of 110 pregnant women undergoing cesarean delivery who were admitted to our hospital from February 2016 to December 2017 were randomly selected as the study subjects. They were divided into ropivacaine group and bupivacaine group by parity grouping, with 55 cases in each group. The ropivacaine group received a single spinal anesthesia with ropivacaine and the bupivacaine group received a single spinal anesthesia with bupivacaine, comparing the maternal, anesthetic effects, and adverse events. Results The onset time of sensory block, the time to reach the highest block plane and the onset time of motor block in the ropivacaine group were (73.01±14.13) s, (6.72±1.14) min and (2.64±0.63) min, respectively,they were slower than the bupivacaine group (59.10±8.06) s, (5.51±1.42) min, and (2.12±0.44) min,the difference was statistically significant (P<0.05). The complete recovery time of the ropivacaine group was (2.12±0.43) h, which was faster than that of the bupivacaine group (2.61±0.53) h,the difference was statistically significant (P<0.05). The incidence of adverse reactions in the bupivacaine group was 9.09%, which was higher than 23.64% in the ropivacaine group the difference was statistically significant (P<0.05). Conclusion In the combined spinal-epidural block, compared with bupivacaine, the clinical effect of ropivacaine is more accurate, and the patient's complete recovery time is shorter and the incidence of adverse reactions is lower.
