目的 应用Meta分析评价空心加压螺钉与动力髋螺钉治疗股骨颈骨折对术后股骨头坏死及相关并发症的影响。方法 计算机检索 PubMed、Cochrane Library、Web of Science、CNKI等医学文献数据库所有关于空心加压螺钉与动力髋螺钉治疗股骨颈骨折的随机对照研究或回顾性对照研究文献,截止到2018年10月。采用RevMan 5.3软件进行统计分析。结果 共7篇文献 纳入标准,其中RCT3篇,CCT4篇,Meta 分析结果显示,股骨头坏死率与两种内固定方式无显著差异(OR=0.71,
Abstract:
Objective To evaluate the effects of cannulated compression screws and dynamic hip screws on femoral head necrosis and related complications after treatment of femoral head fractures. Methods Computer search PubMed, Cochrane Library, Web of Science, CNKI and other medical literature databases All randomized controlled trials or retrospective controlled studies of hollow compression screws and dynamic hip screws for the treatment of femoral neck fractures, as of October 2018. Statistical analysis was performed using RevMan 5.3 software. Results A total of 7 articles were included in the standard, including 3 RCT and 4 CCT. The results of meta-analysis showed that there was no significant difference between the femoral head necrosis rate and the two internal fixation methods (OR=0.71, 95% CI: 0.51~1.0, P=0.05). However, the internal fixation failure was statistically significant compared with the two internal fixation methods (OR=1.49, 95% CI: 1.39~1.57, P<0.01). Conclusion Compared with CCS, DHS can significantly reduce the postoperative internal fixation failure rate of patients with femoral neck fracture, and does not increase the rate of femoral head necrosis. This method is recommended for patients with femoral neck fractures.
