摘要:
目的 比较腹腔镜胰十二指肠切除术（LPD）与开腹胰十二指肠切除术（OPD）临床疗效。方法 回顾性分析承德医学院附属医院2015年1月~2018年8月收治的21例行腹腔镜胰十二指肠切除术（LPD）与20例开腹胰十二指肠切除术（OPD）患者围手术期相关资料，比较两组患者手术过程、术后恢复及其他相关围手术期情况。结果 两组患者均顺利完成胰十二指肠切除术。两组患者手术时间和手术出血量比较，差异无统计学意义（P＞0.05）。LPD组与OPD组重症监护时间[（9.32±2.11）d vs（7.34±3.51）d
Abstract:
Objective To compare the clinical efficacy of laparoscopic pancreaticoduodenectomy (LPD) with open pancreaticoduodenectomy (OPD). Methods A retrospective analysis of 21 cases of laparoscopic pancreaticoduodenectomy (LPD) and 20 cases of open pancreaticoduodenectomy (OPD) admitted to the Affiliated Hospital of Chengde Medical College from January 2015 to August 2018 Perioperative data of patients were compared between the two groups in terms of surgical procedure, postoperative recovery and other related perioperative conditions.Results Pancreaticoduodenectomy was successfully performed in both groups. There was no significant difference in the operation time and the amount of surgical bleeding between the two groups (P>0.05). Intensive care time in LPD group and OPD group [(9.32±2.11)d vs (7.34±3.51)d], biliary drainage tube extraction time [(8.61±2.83)d vs (12.42±3.82)d], hospitalization expenses[(9.31±1.81)ten thousand yuan vs (6.92±1.03) ten thousand yuan], hospitalization days [(27.41±5.82) d vs (33.71±7.74) d], the differences were statistically significant (P<0.05). There were no significant differences between the LPD group and the OPD group in the time of ingestion, the number of pain medications, the time of removal of the urinary catheter, the time of removal of the gastric tube, the time of removal of the pancreatic drainage tube and the number of lymph nodes removed (P>0.05). There was no significant difference in the incidence of postoperative complications between the two groups (P>0.05). Conclusion LPD can achieve the same effect as OPD in terms of surgical bleeding volume, radical degree and postoperative complications. LPD is superior to OPD in hospitalization time and postoperative recovery. The clinical efficacy is worthy of recognition.
