摘要:
目的 探讨联合应用尿免疫球蛋白(IgG)+转铁蛋白(TRF)+微量蛋白(mALB)+β2-微球蛋白(β2-MG)检测妊娠期高血压的效果。方法 选取2018年1月~12月我院接收的妊娠期高血压患者320例,按照疾病严重程度分成A组(150例,重度子痫者)、B组(170例,妊娠期高血压者),同时选取健康体检的妊娠妇女160例作为对照组,比较三组尿IgG、TRF、mALB、β2-MG水平及尿检各指标异常率。结果 A组尿IgG、TRF、mALB、β2-MG分别为(108.26±28.89)mg/L、(75.52±25.57)mg/L、(226.63±78.85)mg/L、(1108.36±225.56)mg/L,高于B组的(36.65±12.27)mg/L、(30.18±14.45)mg/L、(64.45±41.19)mg/L、(746.63±172.23)mg/L和对照组的(14.41±5.59)mg/L、(3.25±1.87)mg/L、(12.24±4.86)mg/L、(241.18±88.39)mg/L,差异有统计学意义(P<0.05);B组IgG、TRF、mALB、β2-MG高于对照组,差异有统计学意义(P<0.05)。A组1项异常率、2项异常率、3项异常率低于B组(0 vs 8.24%)、(0 vs 26.47%)、(28.00% vs 39.41%),4项异常率高于B组(72.00% vs 25.88%),差异具有统计学意义(P<0.05);B组3项异常率、4项异常率高于对照组(39.41% vs 0)、(25.88% vs 0),差异具有统计学意义(P<0.05)。结论 随着妊娠期高血压的加重,尿液中IgG、TRF、mALB、β2-MG各项指标也呈逐渐升高的趋势,联合检测尿中各种蛋白可有效诊断妊娠期高血压。
Abstract:
Abstract:Objective To investigate the effect of combined use of urinary immunoglobulin (IgG) + transferrin (TRF) + microprotein (mALB) + β2-microglobulin (β2-MG) in the detection of pregnancy-induced hypertension.Methods 320 patients with hypertensive disorder of pregnancy received from our hospital from January to December 2018 were divided into group A (150 patients with severe eclampsia) and group B (170 patients with hypertensive disorder) according to the severity of the disease. At the same time, 160 pregnant women with healthy physical examination were selected as the control group. The levels of urinary IgG, TRF, mALB, β2-MG and the abnormal rate of urine test were compared.Results Urinary IgG, TRF, mALB, and β2-MG in group A were (108.26±28.89) mg/L, (75.52±25.57) mg/L, (226.63±78.85) mg/L, and (1108.36±225.56) mg/L, respectively. Higher than group B (36.65±12.27) mg/L, (30.18±14.45) mg/L, (64.45±41.19) mg/L, (746.63±172.23) mg/L, and control group (14.41±5.59) mg /L, (3.25±1.87) mg/L, (12.24±4.86) mg/L, (241.18±88.39) mg/L, the difference was statistically significant (P<0.05), group B IgG, TRF, mALB, β2 -MG was higher than the control group,the difference was statistically significant (P<0.05). The abnormal rate, the 2 abnormal rate and the 3 abnormal rate of group A were lower than those of group B (0 vs 8.24%), (0 vs 26.47%), (28.00% vs 39.41%), and the abnormal rate of 4 cases was higher than that of group B. (72.00% vs 25.88%), the difference was statistically significant (P<0.05); the rate of 3 abnormalities and 4 abnormalities in group B was higher than that in the control group (39.41% vs 0), (25.88% vs 0), the difference was statistical significance (P<0.05).Conclusion With the aggravation of hypertension in pregnancy, the indexes of IgG, TRF, mLBB and β2-MG in urine also tend to increase gradually. Combined detection of various proteins in urine can effectively diagnose pregnancy-induced hypertension.
