摘要:
随着医疗水平发展及内镜技术普及，十二指肠胃肠间质瘤（GIST）检出率日渐提高，这一特殊部位的GIST也吸引了更多外科学者的关注。对于原发可切除的十二指肠GIST手术治疗方式，主要包括局限性切除术（LR）及胰十二指肠切除术（PD）。手术方式的选择主要取决于肿瘤系膜侧边缘距乳头的距离以及是否周围器官侵犯。LR的手术方式包括十二指肠楔形切除术、远端胃部分切除术及十二指肠节段切除术，根据肿瘤的大小及部位做出选择。LR手术在近期疗效方面优于PD手术，远期疗效取决于肿瘤自身生物学行为。腹腔镜联合内镜外科技术治疗十二指肠GIST具有微创、精准等优势。本文将对十二指肠GIST的手术选择原则、疗效比较及其相关研究进展进行综述。
Abstract:
Abstract：With the development of medical level and the popularity of endoscopic techniques, the detection rate of duodenal gastrointestinal stromal tumor (GIST) has been increasing. This special part of GIST has attracted more attention from surgeons. For the primary resectable duodenal GIST surgical treatment, mainly including local resection (LR) and pancreaticoduodenectomy (PD). The choice of surgical approach depends primarily on the distance of the lateral edge of the mesentery from the nipple and whether the surrounding organs are invading. LR surgical procedures include duodenal wedge resection, distal partial gastrectomy, and duodenal segmentectomy, depending on the size and location of the tumor. LR surgery is superior to PD surgery in terms of short-term efficacy, and long-term efficacy depends on the biological behavior of the tumor itself. Laparoscopic combined with endoscopic surgery for the treatment of duodenal GIST has the advantages of minimally invasive and precise. This article will review the principles of surgical selection, efficacy comparison and related research progress of duodenal GIST.
