摘要:
目的 探讨血清癌胚抗原相关细胞粘附分子1(CEACAM1)与糖耐量异常(IGT)患者合并颈动脉粥样硬化(CAS)的相关性。方法 选取2015年10月~2017年10月我院收治的IGT患者80例,根据颈动脉彩色多普勒超声结果分为IGT组44例和IGT+CAS组36例,另选取同期我院健康体检者30名作为正常对照组(NGT组),比较三组临床指标(TG、TC、LDL-C等),分析血清CEACAM1与临床指标的相关性及其影响因素。结果 三组年龄、性别、BMI、SBP、DBP比较,差异无统计学意义(P>0.05)。IGT组和IGT+CAS组TG、TC、LDL-C、FBG、HbA1c、FINS、HOMA-IR、hs-CRP、IL-6、TNF-α均高于NGT组,HDL-C低于NGT组,差异有统计学意义(P<0.05);IGT+CAS组TG、TC、LDL-C、FBG、FINS、HOMA-IR、IL-6、TNF-α高于IGT组,HbA1c、HDL-C低于IGT组,差异有统计学意义(P<0.05)。IGT+CAS组、IGT组CEACAM1水平低于NGT组,IGT+CAS组CEACAM1水平低于IGT组,IGT组CEACAM1水平低于NGT组,差异有统计学意义(P<0.05)。Pearson相关性分析表明,IGT患者血清CEACAM1与TC、LDL-C、FBG、HbA1c、FINS、HOMA-IR呈负相关,与HDL-C呈正相关(P<0.05);与年龄、BMI、SBP、DBP、TG、hs-CRP、IL-6、TNF-α无相关性(P>0.05)。多元线性回归结果显示,TC、HDL-C和HbA1c是血清CEACAM1的独立影响因素。结论 CEACAM1可能与IGT的发生发展有关;CEACAM1水平降低,可能是IGT患者CAS发生发展的原因之一。
Abstract:
Abstract:Objective To investigate the correlation between serum carcinoembryonic antigen-associated cell adhesion molecule 1 (CEACAM1) and carotid atherosclerosis (CAS) in patients with impaired glucose tolerance (IGT). Methods 80 patients with IGT admitted to our hospital from October 2015 to October 2017 were enrolled. According to the results of carotid color Doppler ultrasound, 44 patients in the IGT group and 36 patients in the IGT+CAS group were selected. Thirty patients were used as normal control group (NGT group), and three groups of clinical indicators (TG, TC, LDL-C, etc.) were compared. The correlation between serum CEACAM1 and clinical indicators and its influencing factors were analyzed. Results There were no significant differences in age, gender, BMI, SBP and DBP between the three groups (P>0.05). TG, TC, LDL-C, FBG, HbA1c, FINS, HOMA-IR, hs-CRP, IL-6, TNF-α in IGT group and IGT+CAS group were higher than NGT group, and HDL-C was lower than NGT group,the difference was statistically significant (P<0.05);TG, TC, LDL-C, FBG, FINS, HOMA-IR, IL-6, TNF-α in IGT+CAS group were higher than IGT group, HbA1c and HDL-C were lower than IGT group, the difference was statistically significant (P<0.05). The levels of CEACAM1 in IGT+CAS group and IGT group were lower than those in NGT group. The level of CEACAM1 in IGT+CAS group was lower than that in IGT group. The level of CEACAM1 in IGT group was lower than that in NGT group,the difference was statistically significant (P<0.05). Pearson correlation analysis showed that serum CEACAM1 was negatively correlated with TC, LDL-C, FBG, HbA1c, FINS and HOMA-IR in IGT patients (P<0.05), and positively correlated with HDL-C (P<0.05). There was no correlation between age,BMI, SBP, DBP, TG, hs-CRP, IL-6 and TNF-α (P>0.05). Multiple linear regression results showed that TC, HDL-C and HbA1c were independent factors influencing serum CEACAM1.Conclusion CEACAM1 may be involved in the development of IGT. The decrease of CEACAM1 may be one of the reasons for the development of CAS in patients with IGT.
