摘要:
目的 观察不同游离组织皮瓣移植修复口腔颌面部恶性肿瘤切除术后组织缺损的效果。方法 选取2017年12月~2018年12月我院收治的口腔颌面部恶性肿瘤切除术后组织缺损患者106例，按照随机数字表法分为观察组和对照组，各53例。对照组采用前臂游离皮瓣移植修复，观察组采用股前外侧游离皮瓣移植修复，比较两组修复效果、并发症发生率、吞咽功能及生活质量评分。结果 两组修复成功率比较，差异无统计意义（P＞0.05）；观察组修复后并发症总发生率低于对照组，差异有统计学意义（P＜0.05）；观察组修复后3个月内吞咽功能4级占比为43.40%，高于对照组的24.53%，差异有统计学意义（P＜0.05）；观察组修复后生活质量评分为（80.23±5.67）分，高于对照组的（72.34±7.02）分，差异有统计学意义（P＜0.05）。结论 前臂皮瓣、股前外侧皮瓣均能有效修复口腔颌面部恶性肿瘤切除术后组织缺损，但股前外侧游离皮瓣移植修复后的并发症发生率更低，改善吞咽功能效果更显著，且生活质量更佳。
Abstract:
Objective To observe the effect of different free tissue flaps on the repair of tissue defects after oral and maxillofacial malignant tumor resection. Methods A total of 106 patients with tissue defects after oral and maxillofacial malignant tumor resection in our hospital from December 2017 to December 2018 were enrolled. According to the random number table method, the observation group and the control group were divided into 53 cases. The control group was treated with free forearm free flap transplantation. The observation group was treated with anterior lateral femoral free flap transplantation. The repair effect, complication rate, swallowing function and quality of life score were compared between the two groups.Results There was no statistically significant difference in the success rate between the two groups (P>0.05).The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant (P<0.05). The level of swallowing function in the observation group was 43.40% within 3 months after repair, which was higher than that of the control group 24.53%,the difference was statistically significant (P<0.05). The quality of life score of the observation group was (80.23±5.67), which was higher than that of the control group (72.34±7.02),the difference was statistically significant (P<0.05). Conclusion The forearm flap and anterolateral thigh flap can effectively repair the tissue defect after oral and maxillofacial malignant tumor resection, but the complication rate of anterior femoral free flap transplantation is lower, and the effect of improving swallowing function is more significant and the quality of life is better.
