摘要:
目的 分析四维彩超与二维彩超在胎儿畸形诊断中的应用价值。方法 选取2018年1月~2019年1月在我院行产前胎儿畸形筛查的700例孕妇作为研究对象,分别采用二维彩超、四维彩超进行检查,以胎儿出生后的实际情况为标准,比较两种检查方法对胎儿畸形和具体畸形部位的诊断准确率。结果 胎儿出生后的实际诊断53例胎儿畸形,二维彩超正确诊断28例,四维彩超正确诊断49例,四维彩超对胎儿畸形诊断准确率(92.45%)高于二维彩超的(52.83%),且四维彩超对面部畸形、躯干畸形、四肢畸形、心脏畸形、脊柱畸形的诊断准确率分别为86.67%、100.00%、100.00%、87.50%、83.33%,均高于二维彩超的53.33%、61.54%、66.64%、25.00%、50.00%,差异有统计学意义(P<0.05)。结论 四维彩超与二维彩超在胎儿畸形诊断中均有一定的应用价值,但四维彩超对胎儿畸形的诊断准确率更高,临床可优先使用其行产前胎儿畸形筛查诊断。
Abstract:
Objective To analyze the application value of four-dimensional color Doppler ultrasound and two-dimensional color Doppler ultrasound in the diagnosis of fetal malformation. Methods A total of 700 pregnant women who were screened for prenatal gestational malformation in our hospital from January 2018 to January 2019 were enrolled in the study. Two-dimensional color Doppler ultrasound and four-dimensional color Doppler ultrasound were used to examine the actual situation after birth. Compare the accuracy of the two examination methods for the diagnosis of fetal malformations and specific deformities. Results 53 cases of fetal malformation were diagnosed after birth, 28 cases were correctly diagnosed by 2D color Doppler ultrasound, 49 cases were correctly diagnosed by 4D color Doppler ultrasound,the accuracy rate of diagnosis of fetal malformation by 4D color ultrasound (92.45%) is higher than that of 2D color ultrasound (52.83%).The diagnostic accuracy of four-dimensional color Doppler ultrasound for facial deformity, trunk deformity, limb deformity, cardiac malformation and spinal deformity were 86.67%, 100.00%, 100.00%, 87.50%, and 83.33%, respectively,they were higher than 53.33%, 61.54%, 66.64%, 25.00%, and 50.00% of the two-dimensional color ultrasound,the difference was statistically significant (P<0.05). Conclusion Four-dimensional color Doppler ultrasound and two-dimensional color Doppler ultrasound have certain application value in the diagnosis of fetal malformation. However, the diagnostic accuracy of four-dimensional color Doppler ultrasound for fetal malformation is higher. Clinically, the prenatal gestational malformation screening diagnosis can be used preferentially.
