摘要:
目的 分析医务人员血源性职业暴露的危险因素,为制定职业暴露预防控制措施提供可靠依据。方法 通过现场调查和资料分析我院2017年1月~2018年12月188名医务人员职业暴露情况,分析其工龄、职业、科室分布、暴露方式及操作环节、暴露源种类及暴露后处理情况。结果 共188例血源性职业暴露医务人员,高危人群以护士为主、占67.02%;工作年限以<5年的医护人员为主,占45.21%;科室以外科工作的医护人员为主,占47.34%。职业暴露方式以锐器伤为主,占89.89%,其次为黏膜暴露和破损皮肤。职业暴露操作环节以锐器处理不当为主,占45.21%;其次为输液拔针、医疗废物再次分类、其他、器械清洗、缝合、传递手术锐器。暴露源中以HBV感染为主,占29.79%;其次为不明暴露源、梅毒、病原均为阴性、丙肝、HIV。结论 某三级综合医院医务人员血源性职业暴露以护士、工作年限<5年、外科工作者为主,职业暴露方式以锐器伤为主,职业暴露操作环节以锐器处理不当为主,暴露源以HBV感染为主。因此,医院应加强重点人群、重点环节的职业安全教育,合理配置防护用品,建立健全职业暴露防控机制,有效降低职业暴露发生率。
Abstract:
Objective To analyze the risk factors of blood-borne occupational exposure of medical staff and provide a reliable basis for the development of occupational exposure prevention and control measures. Methods Through field investigation and data analysis, the occupational exposure of 188 medical staff in our hospital from January 2017 to December 2018 was analyzed, and their working age, occupation, department distribution, exposure methods and operation links, types of exposure sources, and post-exposure treatment were analyzed. Results A total of 188 blood-borne occupational exposure medical staff, the high-risk group was mainly nurses, accounting for 67.02%; the working years were mainly medical staff with <5 years, accounting for 45.21%; the department was mainly medical staff with surgical work, accounting for 47.34 %. Occupational exposure was mainly sharp injuries, accounting for 89.89%, followed by mucosal exposure and damaged skin. Occupational exposure operations mainly consisted of improper disposal of sharps, accounting for 45.21%; followed by infusion needle removal, reclassification of medical waste, other, instrument cleaning, sutures, and delivery of surgical sharps. The main sources of exposure were HBV infection, accounting for 29.79%; followed by unknown sources, syphilis, pathogens were negative, hepatitis C, HIV. Conclusion The blood-borne occupational exposure of medical staff in a tertiary general hospital is mainly nurses, working years <5 years, and surgeons. The occupational exposure method is mainly sharp injury. The occupational exposure operation is mainly performed by improper sharps. The main source of exposure is HBV infection. Therefore, hospitals should strengthen occupational safety education for key populations and key links, reasonably allocate protective equipment, establish and improve occupational exposure prevention and control mechanisms, and effectively reduce the incidence of occupational exposures.
