摘要:
目的 分析右美托咪啶对脓毒症急性肾损伤患者肾功能及血管紧张素Ⅱ表达的影响。方法 选择2018年3月~2019年3月在我院诊治的脓毒症急性肾损伤患者96例,随机分为对照组和观察组,各48例。对照组采用常规对症治疗,观察组在对照组基础上采用右美托咪啶治疗。比较两组临床肾功能指标[血肌酐(Scr)、尿素氮(BUN)、血清胱抑素C(Cys C)、尿24h肌酐清除率(24h Ccr)]、血管紧张素Ⅱ(Ang Ⅱ)。结果 治疗后两组Scr、BUN、Cys C均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后两组24h Ccr高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);治疗后两组AngⅡ均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论 右美托咪啶治疗脓毒症急性肾损伤,可有效保护肾功能,减少AngⅡ表达,进一步减轻AngⅡ导致的相关损伤。
Abstract:
Objective To analyze the effects of dexmedetomidine on renal function and angiotensin Ⅱ expression in patients with acute kidney injury in sepsis. Methods A total of 96 patients with acute kidney injury with sepsis diagnosed and treated in our hospital from March 2018 to March 2019 were selected and randomly divided into a control group and an observation group, 48 cases each. The control group received conventional symptomatic treatment, and the observation group received dexmedetomidine on the basis of the control group. The clinical renal function indicators [blood creatinine (Scr), urea nitrogen (BUN), serum cystatin C (Cys C), urine 24h creatinine clearance (24h Ccr)], and angiotensin Ⅱ (Ang Ⅱ) were compared between the two groups.Results After treatment, the Scr, BUN, and Cys C of the two groups were lower than before the treatment, and the observation group was lower than the control group, the difference was statistically significant (P<0.05).After treatment, the 24h Ccr in the two groups was higher than before the treatment, and the observation group was higher than the control group, the difference was statistically significant (P<0.05).After treatment, AngⅡ was lower in both groups than before treatment, and the observation group was lower than the control group, the difference was statistically significant (P<0.05).Conclusion Dexmedetomidine in the treatment of acute kidney injury in sepsis can effectively protect renal function, reduce the expression of AngⅡ, and further reduce the related damage caused by AngⅡ.
