摘要:
目的 比较枸橼酸咖啡因和氨茶碱治疗早产儿呼吸暂停(AOP)和预防支气管肺发育不良(BPD)的临床效果。方法 选取2017年9月~2019年9月乳山市人民医院新生儿科收治的80例AOP患儿作为研究对象,采用随机数字表法分为试验组和对照组,每组40例。试验组采用枸橼酸咖啡因治疗,对照组采用氨茶碱治疗,比较两组治疗疗效、症状改善及临床结局。结果 试验组治疗AOP总有效率高于对照组(P<0.05)。试验组AOP总次数、AOP消失时间和恢复出生体重时间优于对照组(P<0.05);两组AOP时间、机械通气时间比较,差异无统计学意义(P>0.05)。两组临床结局比较,差异无统计学意义(P>0.05)。结论 枸橼酸咖啡因治疗AOP疗效确切,可有效缩短AOP总次数、AOP消失时间、恢复出生体重时间,不增加不良反应,安全可靠,但在预防BPD方面还需要大量的临床资料进一步研究。
Abstract:
Objective To compare the clinical effects of caffeine citrate and aminophylline in the treatment of apnea (AOP) and the prevention of bronchopulmonary dysplasia (BPD) in preterm infants. Methods A total of 80 children with AOP who were admitted to the neonatal department of Rushan People’s Hospital from September 2017 to September 2019 were selected as the research subjects. They were divided into experimental group and control group by random number table method, with 40 cases in each group. The experimental group was treated with caffeine citrate, and the control group was treated with aminophylline. The clinical effects and clinical outcomes were compared between the two groups.Results The total effective rate of AOP in the experimental group was 87.50%, which was higher than 60.00% in the control group(P<0.05). The total number of AOP times, AOP disappearance time, and birth weight recovery time in the experimental group were better than those in the control group(P<0.05). There was no significant difference in AOP time and mechanical ventilation time between the two groups (P>0.05). There was no significant difference in the incidence of adverse reactions and clinical outcomes between the two groups(P>0.05). Conclusion Caffeine citrate is effective in treating AOP, which can effectively shorten the total number of AOP, the time of AOP disappearance, the time to restore birth weight, without increasing adverse reactions, and is safe and reliable. However, a lot of clinical data are needed for further research in the prevention of BPD.
