摘要:
目的 评估ICSI显微操作技术在体外助孕Half-ICSI治疗中的有效性。方法 回顾性分析2007年4月~2018年12月于我院接受Half-ICSI治疗的不孕患者中213例周期，同一患者卵子分为两组分别采用IVF和ICSI两种受精方式，比较IVF组和ICSI组的受精情况、胚胎发育潜能、临床妊娠结局及出生缺陷。结果 纳入研究的213个取卵周期共获卵4374枚，平均获卵（19.97±7.53）枚，2159枚卵行IVF，2215枚卵行ICSI，其中成熟卵1840枚。Half-ICSI治疗中，ICSI组的正常受精率（74.62%）高于IVF组（68.18%）（P＜0.05）；两组间卵裂率，优胚率，每周期优质胚胎数比较，差异无统计学意义（P＞0.05），ICSI组异常受精率（1PN率2.28%，3PN率2.23%）低于IVF组（1PN率3.84%，3PN率8.31%）（P＜0.05）。两组种植率、出生缺陷率、累积妊娠率、累积活产率、流产率比较，差异无统计学意义（P＞0.05）；两组活产单胎、活产双胎的体重比较，差异无统计学意义（P＞0.05）。结论 在Half-ICSI治疗中，ICSI可获得较为满意的受精结局，但临床妊娠结局无明显改善。
Abstract:
Objective To evaluate the effectiveness of ICSI micromanipulation technology in Half-ICSI in vitro fertility treatment. Methods A retrospective analysis of 213 cycles of infertility patients who were treated with Half-ICSI in our hospital from April 2007 to December 2018. The eggs of the same patient were divided into two groups using IVF and ICSI respectively.The fertilization status, embryo development potential, clinical pregnancy outcomes and birth defects were compared between the IVF and ICSI groups. Results A total of 4374 eggs were obtained from the 213 egg retrieval cycles included in the study, with an average of (19.97±7.53) eggs, 2159 eggs lined IVF, 2215 eggs lined ICSI, of which 1,840 mature eggs. In Half-ICSI treatment, the normal fertilization rate of ICSI group (74.62%) was higher than that of IVF group (68.18%)(P<0.05). There was no difference in cleavage rate, superior embryo rate, and number of high-quality embryos per cycle between the two groups(P>0.05). The abnormal fertilization rate (1PN rate 2.28%, 3PN rate 2.23%) in the ICSI group was lower than that in the IVF group (1PN rate 3.84%, 3PN rate 8.31%)(P<0.05). There was no significant difference in implantation rate, birth defect rate, cumulative pregnancy rate, cumulative live birth rate, and miscarriage rate between the two groups(P>0.05).There was no significant difference in body weight between single and twin births between the two groups(P> 0.05).Conclusion In Half-ICSI treatment, ICSI can obtain satisfactory fertilization outcomes, but clinical pregnancy outcomes have not improved significantly.
