摘要:
目的 研究胎儿心脏畸形孕早期超声NT厚度及血流频谱分析的应用价值,为临床诊断提供依据。方法 选择2015年1月~2018年1月在永州市中心医院进行产前检查的5000例孕妇作为研究对象。采用美国CE公司生产的E8彩色多普勒超声检测仪对怀孕11~13+6周的孕妇进行检查,与胎儿出生后超声心动图复查结果相互对照,比较胎儿NT增厚、静脉导管血流频谱异常的检测结果对于胎儿心脏畸形诊断的准确性、灵敏度、特异度。结果 胎儿心脏畸形预测方面血流频谱彩色超声检测阳性筛査率为3.44%,阳性预测值为30.23%,特异度为96.56%,阳性预测值为30.23%;NT值增厚阳性筛查率为4.34%,阳性预测值为23.96%,特异度为95.66%,阳性预测值为23.96%;血流频谱及NT值阳性筛查率为0.90%,阳性预测值为66.67%。结论 多普勒超声检查在胎儿心脏畸形的早期筛查中存在一定优势,可根据早期超声NT厚度及血流频谱分析对孕胎儿心脏畸形进行早期的筛查。
Abstract:
Objective To study the application value of ultrasonographic NT thickness and blood flow spectrum analysis in early pregnancy of fetal cardiac malformation, and to provide basis for clinical diagnosis. Methods A total of 5,000 pregnant women who underwent prenatal examination in Yongzhou Central Hospital from January 2015 to January 2018 were selected as the study subjects. The E8 color Doppler ultrasound detector produced by the American CE company was used to check pregnant women from 11 to 13+6 weeks of pregnancy. The results were compared with the results of fetal echocardiography after birth to compare fetal NT thickening and venous catheter blood flow spectrum. The accuracy, sensitivity, and specificity of abnormal detection results for the diagnosis of fetal cardiac malformations. Results In terms of fetal cardiac malformation, the positive screening rate of color ultrasound detection of blood flow spectrum was 3.44%, the positive predictive value was 30.23%, the specificity was 96.56%, and the positive predictive value was 30.23%; the positive screening rate of thickened NT value was 4.34% The positive predictive value was 23.96%, the specificity was 95.66%, and the positive predictive value was 23.96%. The positive screening rate of blood flow spectrum and NT value was 0.90%, and the positive predictive value was 66.67%. Conclusion Doppler ultrasound has some advantages in early screening of fetal heart malformations. Early screening of pregnant fetal heart malformations can be performed based on early ultrasound NT thickness and blood flow spectrum analysis.
