摘要:
目的 比较单独支撑钢板和支撑钢板联合水平带状钢板治疗胫骨平台后外侧骨折的临床疗效。方法 回顾性分析2013年9月~2018年8月我院收治的37例单纯胫骨平台后外侧劈裂塌陷骨折患者的临床资料,根据内固定方式不同分为A组和B组。A组21例采用单独支撑钢板固定,B组16例采用支撑钢板联合水平带状钢板固定,比较两组手术时间、术中出血量及骨折愈合时间、术后并发症发生率、HSS膝关节功能评分及优良率。结果 A组手术时间短于B组[(133.67±14.21)min vs(162.69±11.34)min]、术中出血量少于B组[(218.71±20.96)ml vs(255.19±25.41)ml],差异有统计学意义(P<0.05);两组骨折愈合时间、术后并发症发生率、HSS膝关节功能评分及优良率比较,差异无统计学意义(P>0.05)。结论 单独支撑钢板内固定或联合水平带状钢板内固定均可有效治疗胫骨平台后外侧骨折,但单独支撑钢板治疗既可保证手术疗效及术后功能恢复,又减少手术时间及术中出血量。
Abstract:
Objective To compare the clinical effect of single support plate and support plate combined with horizontal belt plate in the treatment of posterolateral tibial plateau fractures.Methods The clinical data of 37 patients with simple posterior lateral tibial plateau fracture and collapse fractures admitted to our hospital from September 2013 to August 2018 were retrospectively divided into group A and group B according to different internal fixation methods. 21 cases in group A were fixed with a single support plate, and 16 cases in group B were fixed with a support plate combined with a horizontal belt plate. The operation time, intraoperative blood loss and fracture healing time, postoperative complication rate, HSS knee function score and excellent rate were compared between the two groups.Results The operation time of group A was shorter than that of group B [(133.67±14.21) min vs (162.69±11.34) min], the intraoperative blood loss was less than group B [(218.71±20.96) ml vs (255.19±25.41) ml], the difference was statistically significant (P<0.05); there was no statistically significant difference in fracture healing time, incidence of surgical complications, HSS knee function score and excellent rate between the two groups (P>0.05).Conclusion The single support plate internal fixation or combined horizontal belt plate fixation can effectively treat the posterolateral fracture of the tibial plateau, but the single support plate treatment can ensure the surgical efficacy and postoperative functional recovery, and reduce the operation time and intraoperative bleeding.
