摘要:
目的 比较经尿道前列腺电切术（TURP）与经尿道双极等离子电切术（TUVP）治疗良性前列腺增生的疗效。方法 选择2018年6月~2019年6月在我院治疗的82例良性前列腺增生患者，采用随机数字表法分为对照组和观察组，各41例。对照组采用TURP，观察组采用TUVP，比较两组手术时间、术中出血量、留置尿管时间、前列腺症状评分、尿动力学指标[最大尿流率（Qmax）、最大膀胱容量（VMCC）、残余尿量（PVR）]及并发症发生情况。结果 观察组手术时间、术中出血量、留置尿管时间均低于对照组（P＜0.05）；两组术后前列腺评分均低于术前，且观察组低于对照组（P＜0.05）；观察组术后Qmax、VMCC高于对照组，PVR低于对照组（P＜0.05）；术后观察组并发症发生率为4.87%，低于对照组的14.63%（P＜0.05）。结论 TUVP治疗良性前列腺增生疗效确切，较TURP手术时间短，术中出血量少，术后前列腺恢复好，且并发症发生率低。
Abstract:
Objective To compare the efficacy of transurethral resection of prostate (TURP) and transurethral bipolar plasma resection (TUVP) in the treatment of benign prostatic hyperplasia. Methods A total of 82 patients with benign prostatic hyperplasia who were treated in our hospital from June 2018 to June 2019 were selected and divided into a control group and an observation group with a random number table method, 41 cases each. TURP was used in the control group and TUVP was used in the observation group. The operation time, intraoperative blood loss, indwelling catheter time, prostate symptom score, urodynamic index [maximum urinary flow rate (Qmax), maximum bladder capacity (VMCC) Residual urine volume (PVR)] and the occurrence of complications.Results The operation time, intraoperative blood loss, and indwelling catheter time of the observation group were all lower than that of the control group(P<0.05); the postoperative prostate scores of the two groups were lower than before the operation, and the observation group was lower than the control group(P<0.05); postoperative Qmax and VMCC were higher in the observation group than the control group, and PVR was lower than the control group(P<0.05); the incidence of complications in the observation group after operation 4.87%, lower than 14.63% of the control group(P<0.05).Conclusion TUVP has a definite curative effect on the treatment of benign prostatic hyperplasia. It has a shorter operation time than TURP, less intraoperative blood loss, good postoperative prostate recovery, and a low incidence of complications.
