摘要:
目的 探讨支气管镜支气管肺泡灌洗治疗重症肺炎合并肺不张的效果。方法 选取我院2018年6月~2019年6月儿童重症肺炎合并肺不张患儿85例为观察对象,按照随机数表法分为对照组42例和观察组43例。对照组给予经鼻同步间歇正压通气治疗,观察组给予支气管镜支气管肺泡灌洗治疗,比较两组疗效、血气指标[氧分压(PaO2)、二氧化碳分压(PaCO2)、氧合指数(PaO2/FiO2)]及血清CRP、D二聚体(D-D)水平。结果 观察组总有效率为95.35%,高于对照组的80.95%(P<0.05);治疗后观察组PaO2、PaO2/FiO2高于对照组,PaCO2低于对照组(P<0.05);治疗后观察组血清CRP、D-D水平低于对照组(P<0.05)。结论 支气管镜支气管肺泡灌洗治疗小儿重症肺炎合并肺不张能够改善患儿血气,提高疗效,其机制可能与降低血清CRP、D-D有关。
Abstract:
Objective To investigate the effect of bronchoscopic bronchoalveolar lavage in the treatment of severe pneumonia with atelectasis. Methods A total of 85 children with severe pneumonia and atelectasis in children from June 2018 to June 2019 in our hospital were selected as the observation objects. They were divided into 42 cases in the control group and 43 cases in the observation group according to the random number table method.The control group was given nasal synchronous intermittent positive pressure ventilation therapy, and the observation group was given bronchoscope bronchoalveolar lavage therapy to compare the efficacy, blood gas indexes [oxygen partial pressure (PaO2),carbon dioxide partial pressure (PaCO2), and oxygenation index (PaO2)/FiO2)] and serum CRP and D dimer (D-D) levels.Results The total effective rate of the observation group was 95.35%, which was higher than 80.95% of the control group(P<0.05); after treatment, the observation group PaO2, PaO2/FiO2 was higher than the control group, and PaCO2 was lower than the control group (P<0.05);After treatment, the level of serum CRP and D-D in the observation group was lower than that in the control group (P<0.05).Conclusion Bronchoscope bronchoalveolar lavage in the treatment of severe pneumonia and atelectasis in children can improve blood gas and improve the curative effect of children. The mechanism may be related to the reduction of serum CRP and D-D.
