摘要:
目的 分析边缘不光整乳腺癌的超声图像特征。方法 回顾性分析2017年7月~2019年6月我院经病理确诊为乳腺癌的116例患者的临床资料,依据免疫组化检测结果将乳腺癌分为Luminal A型29例、Luminal B型53例、HER2型16例和基底样型18例,比较不同亚型乳腺癌患者一般资料与灰阶超声特征、剪切波弹性成像特征及乳腺肿块有高回声晕和无高回声晕Emax、S-Emax值。结果 四亚型年龄、肿块大小比较,差异无统计学意义(P>0.05);四亚型病理分型、肿块方位、高回声晕、内部微钙化、后方回声比较,差异有统计学意义(P<0.05);其中HER2型乳腺肿块呈水平位者、肿块内部微钙化高于Luminal A型,Luminal B型肿块有高回声晕者高于Luminal A型和HER2型,基底样型肿块后方回声衰减者多于Luminal A型,差异有统计学意义(P<0.05);其余组间比较,差异无统计学意义(P>0.05)。HER2型肿块Emean、Emax、S-Esd大于其他三型,HER2型肿块S-Emax大于Luminal A型和基底样型,HER2型肿块S-Emean大于Luminal A型,HER2型肿块Esd大于Luminal A型和Luminal B型,基底样型Emin、S-Emin大于其他三型,差异有统计学意义(P<0.05);其余组间比较,差异无统计学意义(P>0.05)。四亚型间有高回声晕和无高回声晕Emax、S-Emax比较,差异均无统计学意义(P>0.05)。结论 联合边缘不光整肿块的方位、内部微钙化及弹性特征等超声征象,可进一步识别各亚型乳腺癌的特点。
Abstract:
Objective To analyze the characteristics of ultrasound images of breast cancer with irregular edge.Methods Retrospectively analyzed the clinical data of 116 patients with pathologically diagnosed breast cancer from July 2017 to June 2019 in our hospital. According to the results of immunohistochemistry, breast cancer was divided into 29 cases of Luminal A type and 53 cases of Luminal B type, 16 cases of HER2 type and 18 cases of basal-like type,the general data of patients with different subtypes of breast cancer were compared with the characteristics of gray-scale ultrasound, shear wave elastography, and the Emax and S-Emax values of breast masses with and without high echo halo.Results There was no statistically significant difference in age and size of the four subtypes (P>0.05); the pathological classification of the four subtypes, the location of the mass, the high echo halo, the internal microcalcification, and the comparison of the posterior echoes were statistically significant(P<0.05); where the HER2 type breast mass is horizontal, the microcalcification inside the mass is higher than Luminal A type; the Luminal type B mass with high echo halo is higher than the Luminal type A and HER2 type; the echo attenuation of the basal-like mass is more for Luminal A type, the difference was statistically significant(P<0.05); compared with the other groups, the difference was not statistically significant(P>0.05). HER2 type masses Emean, Emax, S-Esd are greater than the other three types, HER2 type masses S-Emax are greater than Luminal A type and basal-like type, HER2 type masses S-Emean are greater than Luminal A type, HER2 type mass Esd is greater than Luminal A type Luminal B type, basal-like Emin and S-Emin were greater than the other three types, the difference was statistically significant (P<0.05); compared between the other groups, the difference was not statistically significant (P>0.05). There was no significant difference in Emax and S-Emax between the four subtypes with and without hyperechoic halo (P>0.05).Conclusion Ultrasound signs such as the orientation, internal micro-calcification and elasticity characteristics of the marginal dullness mass can further identify the characteristics of various subtypes of breast cancer.
