摘要:
目的 探讨六胺银、PAS、革兰染色和抗酸染色4种特殊染色在泪道放线菌感染诊断中的应用。方法 收集2010年12月~2019年6月复旦大学附属眼耳鼻喉科医院眼科收治的泪道放线菌感染患者30例,分析该病六胺银、PAS、革兰染色、抗酸染色4种特殊染色方法的镜下表现。结果 常规HE染色镜下下可见散在菌落样结构,菌丝结构不清,染色浅淡;六胺银、PAS和革兰染色3种特殊染色方法,镜下均可见放线菌分别呈棕黑色、紫红色和紫蓝色,呈阳性染色;并于低倍镜下可见放线菌菌落呈典型的菊花样外观,高倍镜下可见放线菌菌丝。抗酸染色示,镜下所有组织细胞均呈蓝色,无特异性呈阴性染色。结论 六胺银、PAS、革兰染色及抗酸染色的联合应用有助于泪道放线菌感染的诊断及鉴别诊断。
Abstract:
Objective To explore the application of four special stains of hexaamine silver, PAS, Gram stain and acid-fast stain in the diagnosis of lacrimal actinomycete infection.Methods A total of 30 patients with lacrimal actinomyces infection admitted to the Department of Ophthalmology, Otolaryngology and Affiliated Hospital of Fudan University from December 2010 to June 2019 were collected, the microscopic performance of the disease with four special staining methods including hexaamine silver, PAS, Gram stain and acid-fast stain was analyzed.Results Under the conventional HE staining microscope, scattered colony-like structures can be seen, the mycelium structure is unclear, and the staining is light; There are three special staining methods for hexaamine silver, PAS and Gram staining, and the actinomycetes can be seen under the microscope as brown-black, purple-red and purple-blue, with positive staining; The actinomycete colonies can be seen under a low magnification lens with a typical chrysanthemum-like appearance, and the actinomycete hyphae can be seen under a high power microscope. Acid-fast staining showed that all tissue cells were blue under the microscope, and there was no specific negative staining.Conclusion The combined application of hexaamine silver, PAS, Gram stain and acid-fast stain is helpful for the diagnosis and differential diagnosis of lacrimal actinomycete infection.
