摘要:
目的 评估PDCA质量管理工具加强医疗废物规范化管理的效果,并对其进行成本效益分析,为医院垃圾处置提供参考依据。方法 2019年5~7月选取我院心胸外科、肝胆外科、泌尿外科、血管外科、创伤骨科、耳鼻喉科6个科室作为活动参与科室,应用PDCA循环,专项调研各科室垃圾混放问题,利用鱼骨图分析原因,制定并落实整改措施,对活动前后各科室垃圾混放频次、医疗废物平均产生量、成本-效益结果进行对比。结果 活动开展后,各科室中位垃圾混放频次由16次/周降至6.5次/周,差异有统计学意义(P<0.05);各科室平均医疗废物产生量由(0.237±0.088)kg/(床·d)降至(0.201±0.072)kg/(床·d),差异有统计学意义(P<0.05);经测算活动总成本支出C总=1904元,获取的经济效益E总=4468.2元,在满足各项成本之后仍能获得经济效益2564.2元。结论 PDCA质量管理可以增强医护人员正确进行垃圾分类的意识,有效促进医疗废物减量化,增加可回收垃圾循环利用机会,有助于实现医疗垃圾的“减量化、资源化、无害化”。
Abstract:
Objective To evaluate the effectiveness of the PDCA quality management tool to strengthen the standardized management of medical waste, and conduct a cost-benefit analysis to provide a reference for hospital waste disposal. Methods From May to July 2019, we selected 6 departments of cardiothoracic surgery, hepatobiliary surgery, urology, vascular surgery, traumatic orthopedics, and otolaryngology in our hospital to participate in the activities. The PDCA cycle was used to investigate the garbage mixing problem in each department. The fishbone diagram analyzes the reasons, formulates and implements rectification measures, and compares the frequency of garbage mixing in each department before and after the activity, the average amount of medical waste generated, and the cost-benefit results.Results After the activity was launched, the frequency of median waste mixing in each department was reduced from 16 times/week to 6.5 times/week, the difference was statistically significant (P<0.05); the average amount of medical waste generated by each department was (0.237±0.088) kg/ (bed·d) reduced to (0.201±0.072) kg/(bed·d), the difference was statistically significant (P<0.05); the total cost of activities calculated and calculated C total = 1904 yuan, the total economic benefits obtained E total = 4468.2 yuan, after meeting various costs can still obtain economic benefits of 2564.2 yuan.Conclusion PDCA quality management can enhance the awareness of medical staff to correctly classify waste, effectively promote the reduction of medical waste, increase the recycling opportunities of recyclable waste, and help achieve the "reduction, recycling, and harmlessness" of medical waste.
