摘要:
目的 调查女性高危型人乳头瘤病毒(HR-HPV)及其早期基因(HPVE6/E7mRNA)感染情况,探究其与阴道微生态的关系。方法 收集2018年7月~2020年6月于河池市人民医院妇科就诊的HPV病毒感染女性1000例,根据生殖道HR-HPV和HPV E6/E7mRNA实验室检测结果,分为对照组1(HR-HPV阳性、HPV E6/E7mRNA阴性)300例,对照组2(HR-HPV阴性、HPV E6/E7mRNA阳性)300例,实验组(HR-HPV和HPV E6/E7mRNA均阳性)400例;另选取我院同期体检健康妇女15例,作为空白组。分析四组HR-HPV、HPVE6/E7mRNA感染情况及HR-HPV感染与阴道微生态的关系。结果 1000例HR-HPV阳性患者中单一型感染821例高于复合型感染179例,单一型感染以16型、52型、58型、51型占比最多,复合型感染以16+18型占比最多;空白组未检出假菌丝阴道炎(VVC)、滴虫阴道炎(TV)、需氧菌阴道炎(AV),对照组1、2与实验组的VVC、TV、AV检出率比较,差异无统计学意义(P>0.05);四组清洁度异常、pH值异常、细菌性阴道炎(BV)、乳酸菌产H2O2阳性、菌群密度阳性、菌群多样性阳性的检出率比较,空白组<对照组1<对照组2<实验组,差异有统计学意义(P<0.05)。结论 HR-HPV单一型感染以16型、52型、58型、51型占比最多,复合型感染以16+18型占比最多,感染HR-HPV女性存在阴道微生态异常情况,与BV、乳酸菌异常、菌群密度及多样性改变有关。
Abstract:
Objective To investigate the infection of female high-risk human papillomavirus (HR-HPV) and its early genes (HPVE6/E7mRNA), and explore its relationship with vaginal microbiota.Methods A total of 1,000 HPV-infected women who attended the Department of Gynecology, Hechi People’s Hospital from July 2018 to June 2020 were collected. According to the laboratory test results of HR-HPV and HPV E6/E7 mRNA in the reproductive tract,they were divided into control group 1 (HR-HPV positive, HPV E6/E7mRNA negative) 300 cases, control group 2 (HR-HPV negative, HPV E6/E7mRNA positive) 300 cases, experimental group (HR-HPV and HPV E6/E7mRNA both positive) 400 cases; 15 cases of healthy women in our hospital during the same period of physical examination were selected as the blank group. Analyze the four groups of HR-HPV, HPVE6/E7mRNA infection and the relationship between HR-HPV infection and vaginal microecology.Results Among 1000 HR-HPV-positive patients, 821 had single infections and 179 had multiple infections. Single infections accounted for the largest proportion of type 16, 52, 58, and 51, and multiple infections accounted for 16+18 most; no pseudohyphatic vaginitis (VVC), trichomonas vaginitis (TV), aerobic vaginitis (AV) were detected in the blank group, and the detection rates of VVC, TV, AV in the control group 1, 2 and the experimental group comparison, the difference was not statistically significant (P>0.05); the four groups have abnormal cleanliness, abnormal pH, bacterial vaginitis (BV), lactic acid bacteria producing H2O2 positive, flora density, and flora diversity detection rate in comparison, the blank group
