摘要:
目的 通过Meta分析妇科手术后患者发生深静脉血栓的危险因素，为预防及降低妇科手术后患者发生深静脉血栓提供依据。方法 通过计算机检索PubMed、EMbase、Cochrane Library、Web of Science、中国生物医学文献数据库（CBM）、中国知网数据库、万方数据库、维普中文期刊全文数据库等，检索期限均从各数据库建库至2020年8月16日，收集有关妇科手术后患者深静脉血栓形成危险因素的病例对照研究及队列研究。采用Review Manager5.3进行Meta分析。结果 共纳入9篇文献，12305例患者，其中血栓组632例，对照组11673例。危险因素合并OR值分别为：年龄≥50岁1.45（95%CI：1.09~1.93）、体重指数1.59（95%CI：1.34~1.88）、术后连续卧床时间长2.26（95%CI：1.54~3.29）、有合并症4.70（95%CI：2.54~8.72）、合并高血压1.45（95%CI：1.08~1.94）、恶性肿瘤2.53（95%CI：1.79~3.58）、手术时间长2.39（95%CI：1.81~3.15）、开腹手术2.63（95%CI：1.70~4.06）。结论 年龄≥50岁、体重指数、术后连续卧床时间长、有合并症、合并高血压、恶性肿瘤、手术时间长及开腹手术是妇科手术后患者发生深静脉血栓的危险因素。医务人员应对可改变的因素进行早期干预，加强围手术期的管理。
Abstract:
Objective Meta-analysis of the risk factors of deep vein thrombosis in patients after gynecological surgery provides evidence for preventing and reducing deep vein thrombosis in patients after gynecological surgery.Methods PubMed, EMbase, Cochrane Library, Web of Science, China Biomedical Literature Database (CBM), China HowNet Database, Wanfang Database, Weipu Chinese Journal Full-text Database, etc. were searched by computer.The search period is from the establishment of each database to August 16, 2020, to collect case-control studies and cohort studies on risk factors of deep vein thrombosis in patients after gynecological surgery.Use Review Manager5.3 for Meta analysis.Results A total of 9 articles were included, including 12305 patients, including 632 in the thrombosis group and 11673 in the control group. The combined OR values of the risk factors were: age ≥50 years old,1.45 (95%CI: 1.09~1.93), body mass index 1.59 (95%CI: 1.34~1.88), and continuous bed rest time after surgery 2.26 (95%CI: 1.54~ 3.29), 4.70 with complications (95%CI: 2.54~8.72),complicated with hypertension 1.45 (95%CI: 1.08~1.94), malignant tumor 2.53 (95%CI: 1.79~3.58), long operation time 2.39 (95%CI: 1.81~3.15), laparotomy 2.63 (95%CI: 1.70~4.06).Conclusion Age ≥50 years, body mass index, long continuous bed time after surgery, complications, hypertension, malignant tumors, long operation time and open surgery are risk factors for deep vein thrombosis after gynecological surgery.Medical staff should carry out early intervention for the changeable factors and strengthen the management of the perioperative period.
