摘要:
目的 系统评价倍他司汀联合盐酸氟桂利嗪治疗耳鸣的效果。方法 计算机检索PubMed、The Cochrane Library、Clinical Queries、CNKI、WanFang Data、维普数据库中倍他司汀联合盐酸氟桂利嗪治疗耳鸣的文献,以“倍他司汀、盐酸氟桂利嗪、耳鸣、Flunarizine hydrochloride 、Betahistine、Tinnitus”为检索关键词,检索时限从建库至2020年5月。根据设定的纳入与排除标准用NoteExpress筛选合格的文献并采用RevMan5.3软件进行Meta分析。结果 共计纳入9篇文献包括治疗组和对照组人群各417例。倍他司汀联合盐酸氟桂利嗪治疗耳鸣的总有效率高于对照组[OR=5.25,95%CI(2.33,11.82),P<0.0001],耳鸣严重程度评分低于对照组[MD=-1.96,95%CI(-2.17,-1.76),P<0.00001],听阈水平低于对照组[MD=-5.87, 95%CI(-8.12,-3.63),P<0.00001],血浆粘度120比率低于对照组[MD=-0.29,95%CI(-0.41,-0.17),P<0.00001],全血粘度低切低于对照组[MD=-2.54,95%CI(-3.58,-1.51),P<0.00001]。结论 倍他司汀联合盐酸氟桂利嗪治疗耳鸣较单纯使用盐酸氟桂利嗪安全有效,对于早期原因不明的主观性耳鸣患者,可以采用二者联合进行治疗,同时受纳入文献质量与样本量的影响,建议开展更高质量的研究,以进一步佐证倍他司汀联合盐酸氟桂利嗪治疗耳鸣的效果。
Abstract:
Objective To systematically evaluate the efficacy of betahistine combined with flunarizine hydrochloride in the treatment of tinnitus.Methods Computer search of PubMed, The Cochrane Library, Clinical Queries, CNKI, WanFang Data, Weipu database for betahistine combined with flunarizine hydrochloride in the treatment of tinnitus literature,using "betahistine, flunarizine hydrochloride, tinnitus, Flunarizine hydrochloride, Betahistine, Tinnitus" as search keywords,the search time limit is from the establishment of the database to May 2020.According to the set inclusion and exclusion criteria, NoteExpress was used to screen qualified documents and RevMan5.3 software was used for Meta analysis.Results A total of 9 references were included, including 417 patients in the treatment group and 417 patients in the control group. Betahistine combined flunarizine hydrochloride in the treatment of tinnitus in the total effective rate was higher than the control group[OR=5.25,95%CI=(2.33,11.82),P<0.0001], tinnitus severity score was lower than the control group[MD=-1.96,95%CI=(-2.17,-1.76),P<0.00001], auditory threshold level was lower than the control group[MD=-5.87,95%CI=(-8.12,-3.63),P<0.00001], Plasma viscosity ratio 120 was lower than the control group[MD=-0.29,95%CI=(-0.41,-0.17),P<0.00001], whole blood low shear viscosity was lower than the control group[MD=-2.54,95%CI=(-3.58,-1.51),P<0.00001]. Conclusion Betahistine combined with flunarizine hydrochloride in the treatment of tinnitus is safer and more effective than flunarizine hydrochloride alone. For patients with subjective tinnitus of unknown early cause, the combination of the two can be used for treatment, and it is also subject to the quality of the included literature and sample size. Therefore, it is recommended to carry out higher quality research to further confirm the effect of betahistine combined with flunarizine hydrochloride in the treatment of tinnitus.
