摘要:
目的 探讨游离旋髂浅动脉穿支皮瓣在修复手足皮肤和组织缺损中的术式和临床疗效。方法 回顾性分析2018年6月~2019年12月收治的23例手和足部皮肤及组织缺损的患者，采用一期清创并行VAC覆盖术，二期选用游离旋髂浅动脉穿支皮瓣修复创面，皮瓣供区一期直接缝合。结果 23例皮瓣均存活，未发生血管危象。其中7例出现张力性水泡（换药后自愈），皮瓣及供区的最终都在2周拆线。8例患者在术后半年再次行整形修薄手术。手术时间为（108.39±15.72）min，住院天数为（19.34±4.38）d，术后随访为6~12个月，创面得到了有效修复，供受区伤口均一期愈合，供区呈线性瘢痕，且没有瘢痕挛缩。皮瓣质地柔软，外形也较好。结论 采用游离旋髂浅动脉穿支皮瓣修复手足部皮肤软组织缺损，供区隐蔽，疗效可靠，是一种安全有效的治疗方案。
Abstract:
Objective To explore the surgical method and clinical effect of free superficial circumflex iliac artery perforator flap in repairing hand and foot skin and tissue defects.Methods A retrospective analysis of 23 patients with hand and foot skin and tissue defects admitted from June 2018 to December 2019 were performed with one-stage debridement and VAC coverage.In the second stage, free superficial circumflex artery perforator flaps were used to repair the wound, and the flap donor site was directly sutured in the first stage.Results All 23 skin flaps survived without vascular crisis.7 cases developed tension blisters (self-healing after dressing change), and the flaps and donor site were finally removed within 2 weeks.8 patients underwent plastic and thinning surgery again six months after the operation. The operation time was (108.39±15.72) min, and the hospital stay was (19.34±4.38) d.The postoperative follow-up was 6-12 months. The wounds were effectively repaired, the wounds in the donor and recipient areas healed uniformly, the donor area showed linear scars, and there was no scar contracture. The flap was soft in texture and good in appearance. Conclusion The use of free superficial circumflex iliac artery perforator flaps to repair the skin and soft tissue defects of the hands and feet, the donor site is concealed, and the effect is reliable. It is a safe and effective treatment plan.
